Celem projektu jest wzrost dostępności i jakości usług społecznych zapobiegających ubóstwu i wykluczeniu społecznemu niepełnosprawnych i niesamodzielnych mieszkańców Gminy Zebrzydowice oraz ich opiekunów do 31.12.2018r. Planuje się przez okres 20m-cy objąć wsparciem 23 osoby (15K i 8M).W grupie docelowej znajdzie się: 13 osób (7K i 6M) niesamodzielnych w w.60+ (w tym min. 4K prowadzące samodzielne gospodarstwo domowe),3 osoby (w tym 2K i 1M) niepełnosprawne i 7 biernych zawodowo opiekunów faktycznych osób zależnych (6K i 1M) Wsparcie w projekcie w formie usług społecznych będzie realizowane na terenie Gminy Zebrzydowice.W projekcie planuje się: utworzenie Ośrodka Krótkookresowego Pobytu Dziennego dla 10 niesamodzielnych uczestników/czek, usługi opiekuńcze świadczone w miejscu zamieszkania dla 3 niesamodzielnych uczestników/czek, usługi asystenckie świadczone dla 3 osób z niepełnosprawnościami, działania wspierające 7 opiekunów faktycznych w opiece nad osobami zależnymi (Polish)
