The project assumes social-professional rehab 30 ON(K17,M13) and 20person from the surrounding ON(K15,M5) in MCZ within 01.09.2017-31.12.2018. The subject of the project is the creation of 2 assisted/training apartments (including the Academy of Life) and the short-term intervention stay. The KPI will be provided as part of the apartments (including the AZ).In both forms, an additional room for participants of the KPI will be prepared.The subject of the project comes from the demand at MCZ, for small forms of group housing as an alternative way to counteract the phenomenon of placing dependent persons in the DPS or other care facilities, i.e. counteracting the isolation of these people from the family and the local centre. The service will guarantee the maintenance of living conditions as close to home as possible and maintain family and social ties.It will be adapted to individual needs in order to restore socio-occupational capacity, independence and life resourcefulness of as many project participants as possible. Projekt zakłada rehab.społeczno-zawodową 30 ON(K17,M13) i 20os. z otoczenia ON(K15,M5) w MCz w terminie 01.09.2017-31.12.2018. Przedmiotem proj.jest utworzenie 2mieszkań wspomaganych/treningowych (w tym Akademii Życia) i Krótkoterminowego Pobytu Interwencyjnego. KPI będzie świadczony w ramach mieszkań (w tym w AŻ).W obu formach zostanie przygotowane dodatkowe pomieszczenie dla uczestników KPI.Przedmiot projektu wypływa z zapotrzebowania w MCz,na małe formy mieszkalnictwa grupowego jako alternatywnego sposobu przeciwdziałania zjawisku umieszczania os.niesamodzielnych w DPS lub innych zakładach opieki tj.przeciwdziałania odizolowaniu tych osób od rodziny i środ.lokalnego. Usługa zagwarantuje podtrzymanie warunków życia jak najbardziej zbliżonych do domowych oraz utrzymanie więzi rodzinnych i społecznych.Dostosowana będzie do indywidualnych potrzeb w celu przywrócenia zdolności społeczno-zawodowych, samodzielności i zaradności życiowej jak największej liczby uczestników projektu.
