Przedmiotem projektu jest zapobieganie następstwom chorób cywilizacyjnych, poprzez poprawę dostępności do świadczeń zdrowotnych z zakresu rehabilitacji kardiologicznej, utrwalenie efektów leczenia szpitalnego oraz aktywizację i poprawę jakości życia osób ze schorzeniami układu krążenia starzejącej się populacji regionu. Realizacja projektu zwiększy dostęp do świadczeń zdrowotnych dla pacjentów ze schorzeniami układu krążenia. Zapewniona zostanie kompleksowa, koordynowana opieka dla pacjentów, poprzez świadczenie realizowanej przez multidyscyplinarny zespół specjalistów rehabilitacji, obejmującej leczenie szpitalne, ambulatoryjne, środowiskowe, a także prozdrowotne działania edukacyjne. Jakość świadczonych usług zostanie podniesiona poprzez poprawę warunków lokalowych i wyposażenie w nowoczesny sprzęt, a dostęp do nich poprawiony, dzięki dostosowaniu infrastruktury do potrzeb osób o ograniczonej sprawności.
