Objective of the project: in the period 1.04.2018-30.04.2020 improving the functioning of 90 people (57 women and 33 men) at risk of poverty or social exclusion from 30 families benefiting from social assistance living in the municipality of Smołdzino, in terms of proper performance of care and educational functions through personalised measures to facilitate their access to social services. Project activities: 1.Social services for families – specialist-therapeutic programme for adult family members. 2.Social services for families – specialisation and care programme for children. 3.Environmental Animation. The main results are: 1.Number of places supported in the Programme for the provision of social services existing after the completion of the project: 3. 2.Number of places established in the programme to provide family support and foster care services existing after the completion of the project: 1. 3.Number of persons at risk of poverty or social exclusion covered by social services of general interest in the Programme and covered by family support and foster care services in the programme: 90. Cel projektu: w okresie 1.04.2018-30.04.2020 poprawa funkcjonowania 90 osób (57 kobiet i 33 mężczyzn) zagrożonych ubóstwem lub wykluczeniem społecznym z 30 rodzin korzystających ze wsparcia pomocy społecznej, zamieszkujących na terenie Gminy Smołdzino, w zakresie prawidłowego wypełniania funkcji opiekuńczo-wychowawczych poprzez zindywidualizowane działania ułatwiające tym osobom dostęp do usług społecznych. Grupa docelowa to 90 osób (30 opiekunów osób zależnych/ rodziców i 60 dzieci) z 30 rodzin zagrożonych wykluczeniem społecznym,korzystających ze wsparcia GOPS Smołdzino. Działania proj.: 1.Usługi społeczne dla rodzin – program specjalistyczno-terapeutyczny dla dorosłych członków rodzin. 2.Usługi społeczne dla rodzin – program opiekuńczo-specjalistyczny dla dzieci. 3.Animacja środowiskowa. Główne rezultaty: 1.Liczba wspartych w Programie miejsc świadczenia usług społecznych istniejących po zakończeniu projektu: 3. 2.Liczba utworzonych w programie miejsc świadczenia usług wspierania rodziny i pieczy zastępczej istniejących po zakończeniu projektu: 1. 3.Liczba osób zagrożonych ubóstwem lub wykluczeniem społecznym objętych usługami społecznymi świadczonymi w interesie ogólnym w Programie oraz objętych usługami wspierania rodziny i pieczy zastępczej w programie: 90.
