The project is targeted at 400 inhabitants in the regenerated area (the city centre and its neighbours living around. 22320 persons) at risk of poverty or social exclusion and their families, and in particular to: senior citizens, OZs, helpers and children and young people from such families. The planned actions will focus on 4 key initiatives: Development of care services and specialist services, in the place of residence, including the organisation of feeder services and the provision of telecare, together with the necessary accompanying infrastructure and the creation of a Local Coordination Point providing personalised support to older people and persons with disabilities and stimulating their activity (AON, AUON, animation) -setting up and running a daily support facility (including reworking) -setting up and running a family psychologist with therapeutic treatment. The aim of the project is to create new, sustainable places for the provision of social services and to expand and refine the existing support system, using the infrastructure developed in the regeneration process. The measures taken will help to improve the living standards of older people and to increase or increase the living standards of older people or to increase their autonomy, either with the help of the outside but at home, and the need to stay in institutions providing 24/7 services will be abolished, possibly completely eliminated. In contrast, measures addressed to families will not be fully followed up, will help to improve parental education, reduce the social exclusion of children and young people, thus reducing the number of children in foster care. The actions implemented in the framework of the project will continue with respect to sustainability. The project is integrated into the revitalisation of the 8.1.2-day barriers, bringing together generations. (English)

    Projekt skierowany jest do 400 mieszkańców obszaru rewitalizowanego (centrum miasta wraz z ościennymi dzielnicami, które zamieszkuje ok. 22320 osób)zagrożonych ubóstwem lub wykluczeniem społecznym oraz ich rodzin, a w szczególności do: seniorów, OzN, rodzin charakteryzujących się bezradnością w sprawach opiek.–wych. oraz dzieci i młodzieży pochodzących z takich rodzin. Planowane działania będą dotyczyć 4 głównych inicjatyw: -rozwój usług opiekuńczych i specjalistycznych usług opiek. w miejscu zamieszkania, w tym organizacja dowozu posiłków -rozwój i upowszechnienie teleopieki wraz z niezbędną infrastrukturą towarzyszącą i utworzenie Lokalnego Punktu Koordynującego udzielającego zindywidualizowanego wsparcia seniorom i osobom niepełnosprawnym oraz pobudzającego ich aktywność (AON, AOON, animator) -utworzenie i prowadzenie placówki wsparcia dziennego (w tym streetworking) -utworzenie i prowadzenie rodzinnej poradni psycholog. z mieszkaniem terapeutyczny. Celem projektu jest stworzenie nowych, trwałych miejsc świadczenia usług społecz. oraz rozbudowa i udoskonalenie istniejącego systemu wsparcia, przy wykorzystaniu infrastruktury powstałej w procesie rewitalizacji. Podejmowane działania przyczynią się do podniesienia standardu życia seniorów oraz OzN, wydłużona lub zwiększona zostanie ich samodzielność bądź możliwe będzie funkcjon. przy wsparciu z zewnątrz ale w miejscu zamieszkania, a konieczność pobytu w placówkach zapewniających całodobowe usługi zostanie odsunięta w czasie, ewentualnie całkowicie zniwelowana. Z kolei działania skierowane do rodzin nie w pełni wydolnych wychowawczo, przyczynią się do podniesienia poziomu pedagogizacji rodziców, zahamowany zostanie proces wykluczania społecznego dzieci i młodzieży, co w efekcie ograniczy ilość dzieci kierowanych do pieczy zastępczej. Wdrożone w ramach projektu działania będą kontynuowane w związku z zachowaniem trwałości. Projekt jest zintegrowany z projektem rewitalizacyjnym 8.1.2 Łamiemy bariery, łączymy pokolenia. (Polish)
