| Property / summary: The project involves providing support to 140 people, including 40 dependent persons, 20 carers, and an average of 20 families (80 persons) who have difficulties in fulfilling the function of caring for the area to be rehabilitated (‘Dworkcowa’) in the Municipality of Chojnice in the period 1.01.2018-31.12.2020. Activities will mainly focus on: 1.The elderly and disabled through coverage by specialised and social services, including through the provision of mobile care services and their actual carers. 2. Families facing difficulties in fulfilling their role in caring — educational by means of, inter alia, assistant, therapeutic and psychological support. The project will establish a day-care centre for dependent persons and daily support facilities. In both sections, group activities will take place, individual in the form of a different type of therapy, counselling, leisure activities. (English) / rank | |||

Projekt zakłada objęcie wsparciem 140 osób w tym 40 osób niesamodzielnych, 20 opiekunów faktycznych oraz średnio 20 rodzin (80 osób) mających trudności w wypełnianiu funkcji opiekuńczo - wychowawczych z obszaru objętego rewitalizacją (Dzielnica Dworcowa) w Gminie Miejskiej Chojnice w okresie 1.01.2018 - 31.12.2020. Działania będą skierowane głównie do: 1.Osób starszych i niepełnosprawnych poprzez objęcie ich usługami specjalistycznymi i społecznymi, w tym poprzez świadczenie mobilnych usług opiekuńczych oraz ich opiekunów faktycznych. 2. Rodzin przeżywających trudności w wypełnianiu swoich funkcji opiekuńczo - wychowawczych poprzez pomoc m.in. asystencką, terapeutyczną, psychologiczną. W ramach projektu utworzony zostanie punkt opieki dziennej dla osób niesamodzielnych oraz placówka wsparcia dziennego. W obu punktach odbywać się będą zajęcia grupowe, indywidualne w postaci m.in.różnego rodzaju terapii, poradnictwa, organizacji czasu wolnego. (Polish)
