The project envisages carrying out a pilot implementation of the provision of medical care services carried out by professional medical care professionals. The project will be carried out in three stages. In the first part of the implementation period, a group of 5 candidates for medical care will be recruited to receive training in accordance with the requirements set out in the relevant provisions on the acquisition of professional competences – such training takes place in the form of a two-semester post-secondary or qualifying vocational course (depending on your education). First-line recruitment will include people from the area of operation of the five Care Stations located in: Kolonowski (includes the commune of Kolonowskie), Zagwiździu (Gm. Murów), Kotorzu Małym (Gmina Turawa), Dobrodzień (gm. Dobrodzień) and Otmicach (Gmina Izbicko). There is an interdisciplinary team of nurses, midwives and rehabilitators in each of the Care Stations providing health services to local residents. As part of the second stage, medical caregivers, after obtaining the required competences, will be placed at the disposal of dedicated nurses (one for each of the municipalities) in order to start providing services in accordance with their dispositions and under their direct supervision. The benefits will be addressed to people whose health is caused on average by deep functional shortages (45-80 points on the Barthel scale), while being unable to receive systemic care as part of the NFZ’s services. At the same time, another 5 carers will be recruited and trained to work at the Care Stations (they are in the region of nearly 40) in subsequent municipalities selected according to the needs of the patients and the capabilities of staff. Projekt przewiduje przeprowadzenie pilotażowego wdrożenia świadczenia medycznych usług o charakterze opiekuńczym realizowanego przez profesjonalnych opiekunów medycznych. Projekt przeprowadzony zostanie trzyetapowo. Świadczenia kierowane będą do osób, których stan zdrowia powoduje średnio głębokie niedobory czynnościowe (45-80 pkt w skali Barthel), będąc zarazem niemożliwym do objęcia opieką systemową w ramach świadczeń NFZ.
