Project implemented in the period 01.05.2017-30.06.2019 by NZOZ Health Family s.c. in partnership with Mączka – Gynecologists – partner company, Home Hospice for Children" in Opole, OLIO Gabriela Eliasz-Łukasiewicz and District Health Centre S A in Kluczbork in the northern area (powiats: Keyborski, Brest, Namysłowski, Olleski). The project will support pregnant and postpartum women, newborns and children up to the age of 2 and staff providing health services. Direct result indicators:Number of post-project health services supported by the programme – 34;Number of people at risk of poverty or social exclusion who declare an increase in quality of life through ESF intervention – 470;Product indicators: Number of people at risk of poverty or social exclusion covered by health services in the programme – 470; Number of people covered by health services in the programme – 3050.     Przedmiotem projektu jest poprawa jakości opieki nad kobietą w okresie ciąży, porodu i połogu[wczesne wykrywanie wad rozwojowych,mobilna opieka okołoporodowa,poradnictwo specjalistyczne dla rodziców];Ochrona zdrowia dziecka w kierunku wsparcia jego rozwoju psychospołecznego[pogłębiona diagnostyka rozwoju (psychomotorycznego) dziecka,wsparcie koordynowane,niwelowanie nieprawidłowości w rozwoju dzieci do lat 2] oraz profilaktyka zakażeń pneumokokowych. Działaniem towarzyszącym są działania informacyjno – edukacyjne.Projekt jest odpowiedzią na zdiagnozowane problemy w zakresie:depopulacji w regionie opolskim, niedostatecznej opieki okołoporodowej kobiet w ciąży i po porodzie, oraz niedostatecznej edukacji przyszłych rodziców. Projekt realizowany w okresie 01.05.2017 - 30.06.2019r. przez NZOZ Zdrowa Rodzina s.c w partnerstwie z Mączka - lekarze ginekolodzy - spółka partnerska, Domowym Hospicjum dla Dzieci"w Opolu, OLIO Gabriela Eliasz-Łukasiewicz oraz Powiatowe Centrum Zdrowia S A w Kluczborku na terenie obszaru północnego (powiaty: kluczborski,brzeski,namysłowski, oleski). Wsparciem w ramach projektu zostaną objęte kobiety w ciąży i w okresie połogu, noworodki i dzieci do 2 roku życia oraz personel służb świadczących usługi zdrowotne. Wskaźniki rezultatu bezpośredniego:Liczba wspartych w programie miejsc świadczenia usług zdrowotnych istniejących po zakończeniu projektu - 34;Liczba osób zagrożonych ubóstwem lub wykluczeniem społecznym, które deklarują wzrost jakości życia dzięki interwencji EFS - 470;Wskaźniki produktu: Liczba osób zagrożonych ubóstwem lub wykluczeniem społecznym objętych usługami zdrowotnymi w programie - 470; Liczba osób objętych usługami zdrowotnymi w programie - 3050.
