The project is a response to the growing problems of society in the field of civilisation diseases: overweight, obesity, diabetes. Hence, the aim of the interventions planned in the programme is to reduce the prevalence of overweight, obesity and diabetes by identifying patients and offering them comprehensive support in the form of health services and information and promotion activities. In view of the difficult access to services for the prevention of civilisation diseases and low awareness of their prevention in people at risk of poverty and social exclusion, the NN decided to apply preferences to this group. As a result, the project plans to involve 42 % (294 persons) at risk of poverty or social exclusion. In order to reach and persuade people from the above-mentioned WN group to participate in the project, they will cooperate with OPS and PCPR. In addition, through partnerships with a large medical entity, the NQ will ensure a high standard of health services and respect for patients’ rights (including the right to patient intimacy and dignity). The substantive quality of the activities carried out, including in particular their compliance with medical procedures, will also be the responsibility of the NC Council. The main tasks of the Council will include, inter alia: 1. elaboration of a model of documentation necessary for screening, 2. determination of conditions of qualification of participants for the second stage of the Programme, 3. elaboration of a model of individual programme of the participant, 4. permanent cooperation with the OWM. At the same time, those who, as a result of screening, will be qualified for the next phase of the project, will be covered by comprehensive, modern and suitable tools. The project will be implemented using full compliance with the relevant RPZ. Planned interventions, counteracting threats from civilisation diseases and social exclusion, fit into the objectives of the SSD Programme. (English)

    Realizacja projektu stanowi odpowiedź na narastające problemy społeczeństwa w zakresie chorób cywilizacyjnych: nadwagi, otyłości, cukrzycy. Stąd celem zaplanowanych w nim interwencji jest zmniejszenie częstotliwości występowania nadwagi, otyłości i cukrzycy dzięki identyfikacji osób chorych oraz zaoferowanie im kompleksowego wsparcia w postaci usług zdrowotnych i działań informacyjno-promocyjnych. Mając na uwadze utrudniony dostęp do usług w zakresie profilaktyki chorób cywilizacyjnych i niską świadomość dotyczącą ich zapobiegania u osób zagrożonych ubóstwem i wykluczonych społecznie WN postanowił o zastosowaniu wobec tej grupy preferencji. W rezultacie w projekcie zaplanowano udział osób zagrożonych ubóstwem lub wykluczeniem społecznym na poziomie 42% (294 osoby). W celu dotarcia i namówienie do udziału w projekcie osób z ww. grupy WN nawiąże współpracę z OPS i PCPR. Ponadto WN dzięki podjęciu partnerstwa z dużym podmiotem leczniczym zapewni wysoki standard oferowanych usług zdrowotnych i poszanowanie praw pacjenta (w tym prawo do intymności i godności pacjenta). Za jakość merytoryczną realizowanych działań, w tym przede wszystkim ich zgodność z procedurami medycznymi odpowiadać będzie również powołana przez WN Rada. Do głównych zadań Rady będzie należało między innymi: 1. opracowanie wzoru dokumentacji niezbędnej do przeprowadzenia badań przesiewowych, 2. określenie warunków kwalifikacji uczestników do II etapu Programu, 3. opracowanie wzoru indywidualnego programu uczestnika, 4. stała współpraca z ZWO. Jednocześnie osoby, które w wyniku badań przesiewowych, zakwalifikowane zostaną do kolejnego etapu projektu, objęte zostaną kompleksowym, udzielanym z zastosowaniem nowoczesnych i adekwatnych do ich potrzeb i możliwości narzędzi. Projekt realizowany będzie z zastosowaniem pełnej zgodności z właściwym RPZ. Zaplanowane interwencje, przeciwdziałając zagrożeniom ze strony chorób cywilizacyjnych oraz wykluczeniu społecznemu, wpisują się w cele Programu SSD. (Polish)
