Projekt realizowany w okresie 01.09.2018-30.11.2019 przez MEDSAL SPÓŁKA Z OGRANICZONĄ ODPOWIEDZIALNOŚCIĄ .Projekt dotyczy rozwoju usług zdrowotnych dla osób starszych, w tym z niepełnosprawnościami zamieszkujących na obszarze woj.opolskiego.Projekt realizuje następujące typy projektu:1)Rozwój zdeinstytucjonalizowanych form opieki medycznej nad osobami starszymi, w tym z niepełnosprawnościami,w zakresie m.in.:wsparcia działalności lub tworzenia nowych miejsc opieki medycznej, w tym długoterminowej medycznej opieki domowej, zwiększenia dostępu do usług poprzez zapewnienie niezbędnego transportu. 2) Rozwój działań wpływających na poprawę stanu zdrowia osób starszych,w tym z niepełnosprawnościami,w tym m.in.rehabilitacja ruchowa,psychiatryczna i logopedyczna. Dodatkowo w projekcie świadczone będą usługi społeczne.Niniejszy projekt skierowany jest do osób lub rodzin zagrożonych ubóstwem lub wykluczeniem społecznym, otoczenia osób zagrożonych ubóstwem lub wykluczeniem społecznym oraz personelu służb świadczących usługi zdrowotne zamieszkujących teren woj.opolskiego- w szczególności m.Opola, Brzeg oraz Prószków. Wsparciem zostanie objętych 6 pacjentów(5K i 1M), 10(8K;2M) osób stanowiących personel służb świadczących usługi zdrowotne i społeczne- [rehabilitanci (2K; 1M),lekarz psychiatra oraz lekarz geriatra(1K; 1M),pielęgniarki(3K; 0M)],opiekunki (2K; 0M).W ramach niniejszego projektu zostanie zapewnione świadczenie usług zdrowotnych nad osobami starszymi,w tym z niepełnosprawnościami.
