The project implements the following types of project: 1)Development of care services for dependent persons (...); 2)Development of activities enabling dependent persons to remain in the local community, including actions supporting their own functioning, including, for example, removing architectural barriers at the place of residence, financing the creation of rental equipment (increasing the independence of persons) and care equipment (necessary for care for dependent persons), financing the loan or purchase of this equipment, meal delivery services, transport to the workplace or support centre. 3) Actions to support carers in the care of dependent persons.In addition, the project will provide free health services, going beyond the guaranteed health care provided by qualified personnel directly in the homes of dependent persons. This project is addressed to persons or families at risk of poverty or social exclusion, the environment of people at risk of poverty or social exclusion, and staff providing services in the local community living in the area of the Polish Voivodeship. 8 patients (4K and 4M),3 (3K; 0M) staff of social services – dependent carers (workers), 1 person providing health services (1K); 0M)(working person) and 8 actual carers (6K; 2M) – unemployed. Projekt realizowany w okresie 01.07.2018-30.06.2021 przez Stowarzyszenie Rodzin i Przyjaciół Autystów "Mały Książę" w Kędzierzynie Koźlu działające jako podmiot ekonomii społecznej . Projekt realizuje następujące typy projektu.: 1)Rozwój usług opiekuńczych nad osobami niesamodzielnymi (...); 2)Rozwój działań umożliwiających osobom niesamodzielnym pozostanie w społeczności lokalnej, w tym działania wspierające ich samodzielne funkcjonowanie, w tym np. likwidowanie barier architektonicznych w miejscu zamieszkania, sfinansowanie tworzenia wypożyczalni sprzętu wspomagającego (zwiększającego samodzielność osób) i sprzętu pielęgnacyjnego (niezbędnego do opieki nad osobami niesamodzielnymi), sfinansowanie wypożyczenia lub zakupu tego sprzętu, usługi dowożenia posiłków, przewóz do miejsca pracy lub ośrodka wsparcia. 3) Działania wspierające opiekunów faktycznych w opiece nad osobami niesamodzielnymi.Dodatkowo w projekcie świadczone będą nieodpłatne usługi zdrowotne, wykraczające poza gwarantowane świadczenia opieki zdrowotnej świadczone przez wykwalifikowany personel bezpośrednio w domach osób niesamodzielnych. Niniejszy projekt skierowany jest do osób lub rodzin zagrożonych ubóstwem lub wykluczeniem społecznym, otoczenia osób zagrożonych ubóstwem lub wykluczeniem społecznym oraz personelu służb świadczących usługi w społeczności lokalnej zamieszkujących teren woj.opolskiego. Wsparciem zostanie objętych 8 pacjentów(4K i 4M),3 (3K;0M) osób stanowiących personel służb świadczących usługi społeczne - opiekunowie os. niesamodzielnych (os.pracujące), 1 osoba personelu świadczącego usługi zdrowotne (1K; 0M)(os.pracująca) oraz 8 opiekunów faktycznych (6K;2M) - osoby bezrobotne. W ramach niniejszego projektu zostanie zapewnione świadczenie usług opiekuńczych dla osób niesamodzielnych - osób autystycznych. (Polish)
