The City of Nowy SączMOPS in the NS through the implementation of comprehensive support within the project, in the form of specialised activities providing support in the form of day care and activation of care and care services, activating empowering and supporting dependent residents of Nowy Sącz in the 60th century and their actual caregivers will achieve the main goal Increasing accessibility and quality of social services through the creation of the Daily Senior House. The target group P will be a total of 105 people, including: 60 dependent people, residents of the NS city, aged over 60 years, especially with reduced psychophysical skills, difficulties in meeting life needs, sick, disabled, in difficult mats, lonely, at risk of social exclusion, socially withdrawn, family and 45 actual caregivers of the UP who will receive direct support within the project. P assumes the implementation of adaptation and equipment of the premises in order to create DDS, the implementation of services for each independent UP based on the Individual Action Plan in the form of care services will be provided in the form of care services, activating and empowering and assisting for the whole target group.In addition, the prosthesis will be provided by the service providers.In addition, the provision of services will be optionally addressed to caregivers, family members who are not UP.Independent UP will be supported by staff, i.e. carers, instructors, etc. (English) | |||||||||||||||

Miasto Nowy SączMOPS w NS poprzez realizacje kompleksowego wsparcia w ramach projektuP,w formie specjalistycznych działań zapewniających wsparcie w formie opieki dziennej i aktywizacji usług opiekuńczopielęgnacyjnych, aktywizującousprawniających oraz wspomagającychniesamodzielnych mieszkańców miasta Nowego Sącza w wieku 60oraz ich opiekunów faktycznych osiągnie cel główny Pzwiększy dostępność oraz jakość usług społecznych poprzez utworzenie Dziennego Domu Seniora. Grupą docelową P będzie łącznie 105 osób, w tym: 60 osób niesamodzielnych, mieszkańców miasta NS, w wieku powyżej 60 r.ż, w szczeg. z obniżoną sprawnością psychofizyczną, trudnościami w zaspokajaniu potrzeb życiowych, chorych , niepełnosprawnych, w trudnej syt. mat., osamotnionych, zagrożonych wykluczeniem społ., wycofanych z życia społ.,rodzinnego oraz 45 opiekunów faktycznych UP którzy otrzymają bezpośrednie wsparcie w ramach projektu. P zakłada realizację adaptacji i wyposażenia pomieszczeń w celu utworzenia DDS,realizację usług dla każdego UP niesamodzielnego w oparciu o Indywidualny Plan Działania w formie usług opiekuńczopielęgnacyjnych, aktywizacyjnousprawniających oraz wspomagających dla całej grupy docelowej.Dodatkowo prow. będą działania fakultatywne skierowane do opiekunów, członków rodzin nie będących UP.Niesamodzielni UP będą stale wspierani przez personel tj. opiekunów, instruktora terapii zajęciowej, pracownika socjalnego, psychologa, fizjoterapeutę.Dzięki realizacji P 60 osób zrealizuje IPD zawierający proces aktywizacjikatalog usług, gdzie możliwym będzie osiągnięcie i utrzymanie optymalnego poziomu samodzielności i aktywności w DDS, osiągając tym samym zakładany rezultat P. Dodatkowo 45 UP Opiekunów zreal.zostanie wsparcie edukacyjne, informacyjne, poradnictwo, co stanowiło będzie wzrost kompetencji ww osób. (Polish)
