The project will be implemented in a period of 36 months from March 2016 to February 2019. Main objective of the project: socio-occupational activation of 134 people at risk of poverty or social exclusion in the Proszowice county during the 36 months of implementation of the project, including persons with disabilities, children and young people benefiting from the form of foster care. The selection of the target group – was determined on the basis of the diagnosis of the situation carried out by the beneficiary’s employees in 2015, by analysing the reasons for using the support of PCPR clients and an analysis of the environmental situation in the county. The project assumes a higher proportion of women, which is a response to equalisation of gender barriers and inequalities in the activated area. The project is planned to implement the following actions: Identification of individual needs and potentials of the participant, Implementation of support within the framework of active integration through the implementation of socio-professional activation of project participants, carrying out work of a social character for the benefit of PCPR clients, implementation of professional activation in cooperation with specialised entities selected as part of the competition for project partner. The local activity programme implemented as part of the project will focus on the environmental support of target groups, i.e. people with disabilities and those benefiting from the support of the replacement care system. Environmental actions implemented under the project will be aimed at supporting the employment of disabled people and the development of family forms of foster care. The project will launch social services in the form of assistance services aimed at supporting people with disabilities and families with disabled children. Projekt realizowany będzie w okresie 36 miesięcy od marca 2016 do lutego 2019 roku. Celem główny projektu: aktywizacja społeczno-zawodowa 134 osób zagrożonych ubóstwem lub wykluczeniem społecznym na terenie powiatu proszowickiego w okresie 36 miesięcy realizacji projektu w tym osób niepełnosprawnych, dzieci i młodzieży korzystającej z form pieczy zastępczej. Wybór Grupy Docelowej -określony został na podstawie diagnozy sytuacji przeprowadzonej przez pracowników beneficjenta w roku 2015 poprzez analizę powodów korzystania ze wsparcia klientów PCPR oraz analizę sytuacji środowiskowej na terenie powiatu. Projekt zakłada większy udział liczbowy kobiet, jest to odpowiedź na wyrównywanie barier i nierówności płci na aktywizowanym obszarze. W ramach projektu planowana jest realizacja następujących działań: Identyfikacja indywidualnych potrzeb oraz potencjałów uczestnika, Realizacja wsparcia w ramach aktywnej integracji poprzez realizacji aktywizacji społeczno – zawodowej uczestników projektu, realizację pracy o charakterze socjalnym na rzecz klientów PCPR, realizację aktywizacji zawodowej przy współpracy z wyspecjalizowanymi podmiotami wybranego w ramach konkursu na partnera projektu. Program aktywności lokalnej realizowany w ramach projektu ukierunkowany będzie na środowiskowe wsparcie grup docelowych tj. osób niepełnosprawnych oraz osób korzystających ze wsparcia systemu pieczy zastępczej. Działania środowiskowe realizowane w ramach projektu ukierunkowane będą na wspierania zatrudniania osób niepełnosprawnych oraz rozwój rodzinnych form pieczy zastępczej. W ramach projektu uruchomione zostaną usługi społeczne w formie usług asystenckich ukierunkowanych na wsparcie osób niepełnosprawnych oraz rodzin z dziećmi niepełnosprawnymi. Główny rezultat – efektywność społeczno-zatrudnieniowa uczestników projektu 56% w wymiarze społecznym i 22% w wymiarze zatrudnieniowym. (Polish)
