The target group will consist of 60 people (44K,16M) unemployed persons not registered in the PUP, inactive persons at least 18 persons, persons qualified for third profile (12 persons) of assistance; of which, at least 80 % of people at risk of poverty or social exclusion suffer multiple social exclusion as exclusion due to more than 1 of the grounds, i.e.: • Persons benefiting from social assistance benefits in accordance with the Act of 12.03.2004 on Social Assistance or eligible for social assistance benefits are meeting at least one of the conditions set out in Article 7 of the Act of 12.03.2004, persons benefiting from the Operational Programme Food Aid, • persons qualified for the third aid profile, in accordance with the Act of 20.04.2004r on the promotion of employment and labour market institutions, persons with significant mental disabilities/degreased persons, and moderate, •persons living within the meaning of the Civil Code in the Mazovian Voivodeship in communes below the threshold of defaworisation defined in Mazowiecki barometer of poverty and social exclusion described in point C.1.7 of the proposal. The main objective of the project is to increase the socio-occupational activity of the above-mentioned target group by carrying out the tasks of: (diagnoses of individual needs and potentials of UP, psychological advice, job mediation, effective communication training, personal development workshops, training and internships) from 04.2018 to 07.2019, leading to employment of at least 18 people and progress in social activation of at least 21 people. Grupę docelową stanowić będzie 60 os.(44K,16M)osoby bezrobotne niezarejestrowane w PUP, osoby bierne zawodowo min. 18 osób, osoby zakwalifikowane do III profilu (12osób) pomocy; w tym min. 80% osób zagrożonych ubóstwem lub wykluczeniem społecznym, doświadczających wielokrotnego wykluczenia społecznego rozumianego jako wykluczenie z powodu więcej niż 1 z przesłanek tj.: •osoby korzystające ze świadczeń z pomocy społecznej zgodnie z ustawą z dnia 12.03.2004r.o pomocy społecznej lub kwalifikujące się do korzystania ze świadczeń z pomocy społecznej to jest spełniające co najmniej jedną z przesłanek określonych w artykule 7 ustawy z dnia 12.03.2004r., •osoby korzystające z Programu Operacyjnego Pomoc Żywnościowa, •osoby zakwalifikowane do III profilu pomocy, zgodnie z ustawą z dnia 20.04.2004r.o promocji zatrudnienia i instytucjach rynku pracy, •osoby z niepełnosprawnościami /stopień lekki, umiarkowany, znaczny oraz osoby z zaburzeniami psychicznymi/; •osoby zamieszkujące w rozumieniu Kodeksu Cywilnego na terenie woj.mazowieckiego w gminach poniżej progu defaworyzacji określonego w Mazowieckim barometrze ubóstwa i wykluczenia społecznego opisanych w pkt C.1.7 wniosku. Celem głównym projektu jest podwyższenie aktywności społeczno-zawodowej wyżej wymienionej grupy docelowej poprzez realizację zadań: (diagnozy indywidualnych potrzeb i potencjałów UP,poradnictwa psychologicznego, pośrednictwo pracy, treningu skutecznej komunikacji, warsztatów rozwoju osobistego,szkoleń i staży) od 04.2018r. do 07.2019r. prowadzącą do zatrudnienia minimum 18 osób oraz postępu w aktywizacji społecznej minimum 21 osób. W projekcie zostały opracowane następujące rezultaty: -Liczba osób zagrożonych ubóstwem lub wykluczeniem społecznym, które uzyskały kwalifikacje po opuszczeniu programu-14 osób. -Liczba osób zagrożonych ubóstwem lub wykluczeniem społecznym poszukujących pracy po opuszczeniu programu-34 osoby.
