Cel gł.:Zwiększenie szans na zatrudnienie 40 osób (22k/18m) zagroż. wyklucz.społ., w tym 12 -30% OzN (kryt.dost.5) oraz zapobieganie zjawisku wyklucz.społ. i ubóstwa poprzez aktywizację społ.-zawod. oraz działania wspierające pracodawców w tworzeniu miejsc pracy dla OzN, z terenu dzielnic Wwy Praga Pn., Praga Pd i Targówek (objętych progr.rewitalizacji) - woj.maz. w okresie 01.11.2017-30.04.2019r.Odbiorcami proj.będą os. doświadczające wielokrotnego wyklucz. i os.z zaburz.psych., w tym OzN intelektualną i os.z całościowymi zaburz.rozwojowymi (kryt.mer.szcz.1).Proj.będzie realiz.w partnerstwie podmiotów z 2-ch różnych sektorów: prywat.i społ.(kryt.mer.szcz.2).Do opracowania diagnozy OzN wykorzystane będą zwalidowane produkty finalne wypracowane w ramach proj.innowacyjnych w POKL (kryt.mer.szcz.4).Proj.jest zg.z programem rewitalizacji obowiązującym na obszarze, na którym jest realizowany (kryt.mer.szcz.5) a także przewiduje kompleksowe wsparcie polegające zarówno na aktywizacji społ.–zawod., jak również realiz.działań wspierających pracodawców w tworzeniu miejsc pracy dla OzN (kryt.mer.szcz.6).Wsparcie uczest.odbywa się na podst.IŚR z wykorzystaniem umowy na wzór kontraktu socjal.(kryt.dost.3).Preferowane do objęcia wsparciem będą os.korzystające z POPŻ 2014-20 a zakres wsparcia dla tych os.nie będzie powielał działań, które dana os.otrzymała lub otrzymuje z PO PŻ w ramach działań towarzyszących, o których mowa w PO PŻ (kryt.dost.4).
