The project will be addressed to 60 people(36K/24M)from families at risk of poverty or social exclusion living in WM only in rural areas below the defavorisation threshold. Up will belong to the following subgroups: —persons/families benefiting from social assistance with the Act of 12.03.2004r.o. on social aid or eligible for social aid, i.e. meeting at least 1 of the conditions specified in Article 7 of the above-mentioned Act, -OzN(5 %/3P./2K/1M), -persons eligible for the third aid profile, according to the Act of 20.04.2004 for employment promotion. The aim of the project is to increase socio-occupational activity and higher employability, among 60persons (36K/24M) from families at risk of poverty or social exclusion of the inhabitant.They will be achieved in the period IX 2017-V 2018 through the implementation of comprehensive and individual support for socio-occupational reintegration. TASKS: —Discovery of individual needs and potentials of project participants in order to prepare and implement support based on the path of reintegration, – Psychological advice, pedagogy and law, -Work brokerage, -Coaching supporting family-work balance, -Workshops of social/life competences, -Training and internships. Proj.zostanie skierowany do 60os.(36K/24M)z rodzin zagrożonych ubóstwem lub wykluczeniem społecznym zamieszkałych(w roz.przepisów KC)w WM wyłącznie na obszarach wiejskich poniżej progu defaworyzacji. UP będą należeć do nast.podgrup: -os./rodzin korzystających ze świadczeń z pomocy społ.zg.z ustawą z dnia 12.03.2004r.o pomocy społ.lub kwalifik.się do objęcia wsparciem pomocy społ.,tj.spełniających co najmniej 1 z przesłanek określonych w art.7 ww.ustawy, -OzN(5%/3os./2K/1M), -os.zakwalifikowanych do III profilu pomocy,zgodnie z ustawą z dnia 20.04.2004r.o promocji zatrudn.i instyt.rynku pracy(Dz.U.z 2016r.poz.645,z późn.zm.), -os.korzystających z PO PŻ. CELEM projektu jest zwiększona aktywność społeczno-zawodowa oraz wyższa zdolność do zatrudnienia,wśród 60os.(36K/24M) z rodzin zagrożonych ubóstwem lub wykluczeniem społecznym zamieszk.(w roz.przepisów KC)w WM wyłącznie na obszarach wiejskich poniżej progu defaworyzacji.Zostaną one osiągnięte w okresie IX 2017r.-V 2018r.poprzez realizację kompleksowego i indywidualnego wsparcia w zakresie reintegracji społeczno-zawodowej. ZADANIA: -Diagnoza indywidualnych potrzeb i potencjałów uczestników projektu w celu przygotowania i realizacji wsparcia w oparciu o ścieżkę reintegracji, -Poradnictwo psychologiczne,pedagogiczno-prawne, -Pośrednictwo pracy, -Coaching wspierania równowagi rodzina-praca, -Warsztaty kompetencji społecznych/życiowych, -Szkolenia i staże zawodowe. WSKAŹNIKI: -L.os.zagroż.ubóstwem lub wykluczeniem społecznym,które uzyskały kwalifikacje po opuszczeniu programu-14os. -L.os.zagroż.ubóstwem lub wykluczeniem społecznym poszukujących pracy po opuszczeniu programu–34os. -L.os.zagroż.ubóstwem lub wykluczeniem społecznym,pracujących po opuszczeniu programu (łącznie z pracującymi na własny rachunek)-18os. -Odsetek os.zagrożonych ubóstwem lub wykluczeniem społecznym,które dokonały postępu w aktywizacji społecznej-21os. -Odsetek os.zagrożonych ubóstwem lub wykluczeniem społecznym,które podjęły (Polish)
