Gł.celem proj.jest aktywizacja społeczna,życiowa i zawodowa 55os.zagrożonych wykluczeniem społecznym,zamieszkałych na terenie Pow.Wołomińskiego i korzystających ze wsparcia PCPR Wołomin w okr.od 01.10.2016r. do 31.07.2018r. Uczestnikami proj.będą os.z niepełnosprawnością(20%uczestników),pełnoletni wychowankowie pieczy zastępczej i młodzież powyżej 15roku życia przebywająca w pieczy zastęp.,zagroż.ubóstwem lub wyklucz.społ.z terenu Pow.Wołomińskiego,korzystający ze świadczeń pomocy społ.realizowanej przez PCPR Wołomin.Wsparcie os.odbywać się będzie na podstawie ścieżki reintegracji stworzonej indywidualnie dla każdej os.(indywidual.diagnoza potrzeb i potencjałów uczestników)z uwzględnieniem diagnozy syt.problemowej lub zagrożenia sytuacja problemową,zasobów,potencjału,predyspozycji,potrzeb.Uczestnicy proj.zostaną objęci kontraktami socjalnymi. Zadania: 1.Ścieżka wsparcia dla pełnoletnich wychowanków pieczy zastępczej oraz młodzieży powyżej 15 roku życia przebywającej w pieczy zastępczej. 2.Ścieżka wsparcia dla osób z niepełnosprawnościami.
