Main objective: Increase access to high-quality social services for 55 people at risk of social exclusion, including 40 dependents living in Ciechanów by 30.09.2021. The recipients will be 40 dependent people and 15 members of their families at risk of poverty and social exclusion. People and families covered by the OPŻ The project will be implemented in partnership of entities from different sectors: PCK OR in Ciechanów (entity of social economy) and Ciechanowski County (j.s.t.). There are no links between the partners referred to in Article 33. 6 of the Act of 11 July 2014 on the principles of implementing the threshold for cohesion policy... The project involves the implementation of such forms of support as: environmental care services and specialised services, the establishment of a senior club, which will involve the activation of dependent persons, advisory support for families of non-residents, trainings for non-same families and caregivers. The applicant shall ensure that each time the project leads to an increase in the number of persons supported and an increase in the number of social services provided by a given Applicant in relation to data from the year preceding the year of commencement of the project. Number of places for the provision of care services by the Applicant in 2018: 40. Number of places run by the Applicant in addition to the project: 15. Support in the project will be implemented on the basis of a support path created individually for each participant/center of the project and in accordance with the concept of empowerment. Cel główny: Zwiększenie dostępu do wysokiej jakości usług społecznych dla 55 osób zagrożonych wykluczeniem społecznym, w tym dla 40 osób niesamodzielnych zamieszkałych w Ciechanowie do 30.09.2021r. Odbiorcami pomocy będzie 40 osób niesamodzielnych oraz 15 członków ich rodzin zagrożonych ubóstwem i wykluczeniem społecznym. Wsparciem objęte zostaną osoby niesamodzielne, w szczególności osoby starsze oraz osoby z niepełnosprawnościami, wymagające zapewnienia wsparcia w zaspokajaniu podstawowych potrzeb życiowych oraz procesie leczenia lub rehabilitacji. Preferowane będą osoby i rodziny objęte POPŻ Projekt będzie realizowany w partnerstwie podmiotów z rożnych sektorów: PCK OR w Ciechanowie (podmiot ekonomi społecznej) i Powiatu Ciechanowskiego (j.s.t.). Pomiędzy partnerami brak jest powiązań, o których mowa w art. 33 ust. 6 ustawy z dnia 11 lipca 2014 r. o zasadach realizacji progr. w zakr. polityki spójności... Projekt zakłada realizację takich form wsparcia jak: środowiskowe usługi opiekuńcze i usługi specjalistyczne, utworzenie klubu seniora, w którym prowadzona będzie aktywizacja osób niesamodzielnych, wsparcie doradcze dla rodzin osób niesamodz., szkolenia dla rodzin os. niesam. i opiekunek. Wnioskodawca zapewnia, że projekt prowadzi każdorazowo do zwiększenia liczby osób objętych wsparciem oraz zwiększenia liczby miejsc świadczenia usług społecznych prowadzonych przez danego Wnioskodawcę w stosunku do danych z roku poprzedzającego rok rozpoczęcia realizacji projektu. Liczba miejsc świadczenia usług opiekuńczych przez Wnioskodawcę w 2018r.: 40. Liczba miejsc prowadzonych przez Wnioskodawcę dodatkowo w ramach projektu: 15. Wsparcie w projekcie będzie realizowane na podstawie ścieżki wsparcia stworzonej indywidualnie dla każdego uczestnika/czki projektu oraz zgodnie z koncepcją empowerment. (Polish)
