This project will be implemented in the area of the Prussian district, which in 2016 had a population of 161319 people/84615 women (K), including 175 foster families (RZ) and 69 families were supported by the PCPR due to the disability of the child. They are born in particular situations and report specific needs (especially in part). C3.2), whose condition has a direct impact on the process of deinstitutionalisation of the foster care system – further growth of internal problems of such families threatens a negative assessment, a decrease in the number of places in family care forms and the transition of pupils under the care of PZ institutions. Based on receiving signals from the target group, the PCPR in Pruszków will carry out actions in the period 01.03.2018-31.12.2019 to improve the functioning of a total of 60 families in the area of Pruszków County (target, depth), in which people at risk of social exclusion (person. HSC): resident in replacement care (PZ):40 persons/18K and disabled (ON):20 persons/6K. There will be training for people staying in and holding the PZ and candidates/care holders for PZ, which will create at least 5 new foster families – family forms of foster care in the district. As the participants/casks of the project (UP) will be young (60 people/24K), whose development is strongly, directly influenced by family members, the support will be covered by the environment (95ps./61K). Achieving the next objective by increasing the competence of the target game: social and life, parental and educational, strengthening family ties in the supported families and increasing the qualifications of candidates for foster care – people and families entering a system where deficiencies in the scope and access to community services were diagnosed. The next impact assessment based on quantifiable output and result indicators and the results achieved will be maintained for at least a period equal to the duration of the project. (English)

    Niniejszy projekt zostanie zrealizowany na terenie pow. pruszkowskiego, który w 2016r. zamieszkiwało 161319os./84615 kobiet (K), w tym funkcjonowało 175 rodzin zastępczych (RZ) oraz 69 rodzin było objętych wsparciem PCPR ze wzgl. na niepełnosprawność dziecka. Są to rodz. w szczeg. sytuacji i zgłaszające specyficzne potrzeby (szczeg. w cz. C3.2), których kondycja ma bezpośredni wpływ na proces deinstytucjonalizacji systemu pieczy zastępczej - dalsze narastanie problemów wewn. takich rodzin grozi negatywną oceną, spadkiem liczby miejsc w rodzinnych formach pieczy i przechodzeniem wychowanków pod opiekę instytucji PZ. W oparciu o otrzymanie od grupy docelowej sygnały, PCPR w Pruszkowie przeprowadzi w okresie 01.03.2018-31.12.2019 działania na rzecz poprawy funkcjonowania łącznie 60 rodzin z terenu powiatu pruszkowskiego (cel gł.), w których znajdują się osoby zagrożone wykluczeniem społecznym (os. ZWS): przebywające w pieczy zastępczej (PZ):40 os./18K i niepełnosprawne (ON):20os./6K. Przeszkolone zostaną os. przebywające w i sprawujące PZ oraz kandydaci/tki do sprawowania PZ, dzięki czemu powstanie min. 5 nowych rodzin zastępczych - rodzinnych form pieczy zastępczej na terenie powiatu. Jako że uczestnikami/czkami projektu (UP) będą osoby młode (60 os./24K), na których rozwój mają silny, bezpośrd. wpływ członkowie rodzin, wsparciem zostanie objęte otoczenie (95ps./61K). Osiągnięcie celu nast. poprzez wzrost kompetencji gr. docelowej: społ. i życiowych, rodzicielskich i wychowawczych, wzmocnienie więzi rodzinnych w rodz. objętych wsparciem oraz wzrost kwalifikacji kandydatów/ek do sprawowania pieczy zastępczej - os. i rodziny wkraczające do systemu, w którym zdiagnozowano braki w zakresie i dostępie do usług społ. Ocena efektów nast. w oparciu o policzalne wskaźniki produktu i rezultatu, a osiągnięte wyniki zostaną utrzymane co najmniej przez okres równy okresowi trwania projektu. (Polish)
