The aim of the project is to increase the number of places to provide health services. in the scope of palliate care and support activities by 8 places of the Łódź Voivodeship in the period 2019-04-01-2020-10-31 The project will be covered by 90 non-reliable people suffering from incurable, progressive, life-limiting cancer and non-cancer diseases according to Annex 1 to Rozp. MZ dated 29.10.2013 on guaranteed benefits in the field of paliat care and hospic. The project will be covered by 90 op. factual, learner in training. The total l. student project will be 180 people. As part of the project, the following tasks will be carried out: 1.domestic care palat. for 90 learner. project. The task will be carried out by 18 months at home. The care provision of paliat.i hospic. is a comprehensive, comprehensive care and symptomatic treatment of patients with incurable, untreated causal treatment, progressive, life-limiting disease. Care will be provided 365 days a year. Palliative care will be supplemented by a medical caregiver whose task will be to support the patient in everyday activities. 2. the individual in respect of the patient and his decision-making will as a means of counteracting persistent therapy. The school will improve the process of effective clinical communication, aimed at respecting the patient’s will and discussing early instructions. This is one of the conditions for counteracting persistent therapy (besides early implementation of palliative approach, coordination of health care. and community, achievement of treatment objectives). The training will be attended by carers of all participants of the project. The project will allow the creation of a comprehensive medical and care service, which is home palliative care together with the support of a medical caregiver, so that each of the project participants will be provided care for 7 days in a week, which will contribute to a real increase in safety and will have a direct impact on improving the quality of life of both the patient and the caregiver facts.     Celem proj jest wzrost liczby miejsc świadczenia usług zdrow. w zakresie opieki paliat. i działań wspierających o 8 miejsc woj. łódzkiego w okresie 2019-04-01–2020-10-31 Proj. zostanie objętych 90 os niesamodzielnych chorujących na nieuleczalne,postępujące,ograniczające życie choroby nowotworowe i nienowotworowe zgodnie z Zał. nr 1 do Rozp. MZ z dnia 29.10.2013r. w sprawie świadczeń gwarantowanych z zakresu opieki paliat. i hospic. Projektem zostanie objętych 90 op. faktycznych, ucz. w szkoleniu. Łączna l. ucz. proj. wyniesie 180os. W ramach proj. zostaną zrealizowane zadania z zakresu: 1.domowej opieki paliat. dla 90 ucz. proj. Zadanie będzie realizowane przez 18 m-cy w warunkach domowych. Świadczenia opieki paliat.i hospic. to wszechstronna,całościowa opieka i leczenie objawowe chorych na nieuleczalne,niepoddające się leczeniu przyczynowemu,postępujące,ograniczające życie choroby. Opieka będzie świadczona 365 dni w roku. Opieka paliatywna zostanie uzupełniona przez opiekuna medycznego, którego zadaniem będzie wsparcie chorego w codziennych czynnościach. 2.szk. indywidualne z zakresu uszanowania chorego i jego woli decyzyjnej jako sposób przeciwdziałania uporczywej terapii. Szkol. usprawni proces skutecznej komunikacji klinicznej,ukierunkowanej na uszanowanie woli chorego i przedyskutowanie przedterminowych poleceń. Jest to jeden z warunków przeciwdziałania uporczywej terapii(obok wczesnego wdrożenia podejścia paliatywnego,skoordynowania opieki zdrow. i społ.,urealniania celów leczenia). W szkoleniach udział wezmą opiekunowie wszystkich uczestników projektu. Proj. pozwoli na stworzenie kompleksowej usługi medyczno-opiekuńczej jaką jest domowa opieka paliatywna wraz ze wsparciem opiekuna medycznego, dzięki czemu każdy z uczestników projektu będzie miał zapewnioną opiekę przez 7 dni w tyg., co przyczyni się do realnego zwiększenia bezpieczeństwa i będzie miało bezpośredni ma wpływ na poprawę jakości życia zarówno podopiecznego, jak i opiekuna fakty (Polish)
