Polsce liczba zachorowań na udar mózgu jest porównywalna do statystyk państw Europy Zachodniej.Umieralność z powodu udarów mózgu i powstała po nim niepełnosprawność jest w Polsce znacznie większa.Prowadzone badania epid.dowodzą,że w naszym kraju 30% chorych umiera z powodu udaru mózgu,w czasie pierwszego miesiąca.Z kolei 70% chorych,którzy przeżyli ostry okres udaru mózgu są to osoby niepełnosprawne.W grupie chorych,którzy przeżyli udar mózgu u 1/3 pozostaje duży deficyt funkcjonalny,głównie pod postacią porażenia(niedowładu)połowiczego, afazji,zaburzeń wyższych czynności nerwowych.Chorzy ci wymagają stałej opieki osób drugich.Kolejnym ogromnym problemem w na obszarze województwa łódzkiego jest wysoka skala zachorowalności na choroby psychiczne-w tym z uwagi na proces starzenia się społ.demencję.P. stanowi odpowiedź na te potrzeby.Zakres rzeczowy został określony w wyniku konsultacji ze specj.,którzy posiadają wieloletnie doświadczenie w opiece nad osobami niesamodz.P. skierowany jest do gr.36 osób niesamodz.(19 K i 17 M)oraz ich 36 opiekunów.Celem p.będzie poprawa dostępu do usług zdrow. P. przewiduje zapewnienie dla os. po udarach kompleksowego wsparcia interd.zespołu opieki długoter.oraz stworzenie zespołu środ.op.psychiat.Wsparcie dla osób nies.ma mieć charakter indywidualny tzn.W. zapewni aby plan wsparcia dla poszczególnych os. został stworzony z uwzględnieniem diagnozy syt.zdrow.,rodzinnej,problemowej,potencjałów,predyspozycji i potrzeb.Os.niesamodzielna lub jej opiekun prawny będą uczestniczyć w podejmowaniu decyzji o kształcie,formie i zakresie wsparcia.Główne zadania w projekcie to:pielęgniarska opieka długot.,usługi świadczone w lokalnej społeczności,wsparcie lekarzy spec. wobec niepełnosprawności związanej z przebytym udarem mózgu jak również towarzyszącą wielochorobowością oraz szkolenia dla ich opiekunów.Rezultatem projektu będzie 24 wspartych miejsc świadczenia usług zdrowotnych istn.po zakończeniu projektu.
