The aim of the project is to increase access to high quality community and health services through the creation and operation of specialist care services in the area from 01.01.2019-31.12.2021, the creation of assisted housing, AOON assistance services, transfer services, specialised advice.Social services will be provided for approx. 136 ON K77,M59 Additionally, as part of the project, support will be covered by 65 persons from the environment of dependants/actual caregivers through the creation and operation of the relief service on the area area for caregivers with incomplete intellectual and specialised counselling for op. fact. For each participant in the project, according to the competition, there will be an individual assessment of the material and life situation of the ON, the needs, expectations, the proposed form of support (type and scope). All forms of support will be coordinated at county level in order to provide comprehensive support for people dependent on the local community. In the field of health services we offer palliative and hospice care for home hospice patients.The project assumes 3 tasks: education and health guidance, comprehensive care of the hospice patient and support of families during the course of the illness.The support will translate into a real improvement of the quality of services provided for patients in palliative and hospice care. The planned 3-year duration of the project is to direct support for about 660 ON and 660 people from the ON surroundings. The project supports the transition from institutional care to community-based care. This is done by increasing and supporting the provision of services in the form of services provided in the local environment. Celem projektu jest zwiększenie dostępu do wysokiej jakości usług społ. i zdrowotnych poprzez utworzenie i prowadzenie na terenie pow. zg. w okresie od 01.01.2019-31.12.2021 specjalistycznych usług opiekuńczych, utworzenie mieszkalnictwa wspomaganego,usługi asystenckie AOON, usł.transp,poradnictwo specjalistycznego.Usługi społeczne będą świadczone dla ok. 136 ON K77,M59 Dodatkowo w ramach proj.,wsparciem objętych zostanie 65 os.z otoczenia osób niesamodzielnych/opiekunów faktycznych poprzez utworzenie i prowadzenie na terenie pow.zg.usługi wytchnieniowej dla opiekunów os.z niepeł. intelektualną oraz poradnictwa specjalistycznego dla op. fakt. Dla każdego uczestnika projektu zgodnie z reg. konkursu zostanie opracowana IŚW, zawierająca indywidualną ocenę sytuacji materialnej i życiowej ON, potrzeby, oczekiwania, proponowaną/ne formę/y wsparcia (rodzaj i zakres). Wszystkie formy wsparcia zostaną skoordynowane na poziomie powiatu zg.,tak by zapewnić kompleksowe wsparcie os.zależnym na poziomie lokalnej społeczności. W zakresie usług zdrowotnych oferujemy opiekę paliatywno-hospicyjną dla pacjentów hospicjum domowego.Projekt zakłada realizację 3 zadań:edukację i poradnictwo zdrowotne, kompleksowa opieka pacjenta hospicyjnego oraz wsparcie rodzin w czasie trwania choroby.Wsparcie przełoży się na realne podniesienie jakości świadczonych usług dla pacjentów w opiece paliatywno-hospicyjnej. Założony 3-letni okres trwania projektu zakłada skierowanie wsparcia dla ok.660 ON oraz 660 osób z otoczenia ON. Wsparcie w ramach projektu zakłada przejście od opieki instytucjonalnej do opieki świadczonej na poziomie lokalnych społeczności. Odbywać się to poprzez zwiększenie i wspieranie miejsc świadczenia usług w formach usług świadczonych w środowisku lokalnym. Wsparcie realizowane będzie zgodnie z Wytycznymi w zakresie realizacji przedsięwzięć w obszarze włączenia społecznego i zwalczania ubóstwa z wykorzystaniem środków EFS i EFRR na lata 2014-2020 (Polish)
