CEL PROJEKTU: Wzrost dostępności miejsc dziennego pobytu o 20 w ramach nowo utworzonego Domu Dziennego Pobytu w Paterku oraz wzrost kompetencji 10 osób z otoczenia osób zagrożonych ubóstwem lub wykluczeniem społecznym w obszarze pełnionych funkcji opiekuńczych do 31 maja 2021 r.2. GŁÓWNE PRODUKTY I REZULTATY:Liczba osób zagrożonych ubóstwem lub wykluczeniem społecznym objętych usługami asystenckimi i opiekuńczymi świadczonymi w społeczności lokalnej w programie - 50Liczba utworzonych w programie miejsc świadczenia usług asystenckich i opiekuńczych istniejących po zakończeniu projektu - 203. GRUPA DOCELOWA:I. Grupę docelową projektu będą stanowiły osoby zagrożone ubóstwem lub wykluczeniem społecznym oraz niesamodzielne, tj. osoby, które ze względu na stan zdrowia i niepełnosprawność wymagają opieki lub wsparcia w związku z niemożnością samodzielnego wykonania co najmniej jednej z podstawowych czynności dnia codziennego, zamieszkujące zgodnie z kodeksem cywilnym, obszar powiatu nakielskiego (zgodnie z kryt. B.1.6.). II. Grupę docelową będą stanowiły osoby stanowiące otoczenie osób zagrożonych ubóstwem lub wykluczeniem społecznym (w takim zakresie, w jakim jest to niezbędne dla wsparcia osób wykluczonych społecznie), w tym osoby pełniące obowiązki opiekuńcze.
