Celem proj.jest zwiększ.dostępności niestacj.usług opiek.dla mieszk.gm. Mogilno poprzez zwiększ.l.miejsc świadczenia usług opiekuńczych (UO) i specjalistycznych usług opiekuńczych (SUO) w miejscu zamieszk.w okr.X 2017-VI 2019.Istnieje duże zapotrzebow.na świadczenie UO popołudniami i wieczorami oraz w dni wolne od pracy oraz uruchomienie SUO dla osób z niepełnosprawn.Gr.doc.stanowi 16 osób(13K+3M) w wieku65+,niepełnosprawnych,niesamodz.,korzyst.ze św.z pom. społ.(UO w niewystarczającym zakresie),z probl.w samoobsłudze,samotnych/mających ogranicz.dostęp do pomocy z kręgu rodzinnego,o ograniczonej sprawności ruchu(potrzeba rehab./fizjoter)zam.teren gm.Mogilno(7 os.ze wsi+9 os.z miasta).Proj.przyczyni się do podniesienia poziomu opieki nad w/w osobami i stopniowego rozwiąz.probl.tej grupy. Gł.zadania: -UO świadczone popołudniami,wieczorami oraz w dni wolne od pracy, -SUO świadczone przez wykwalifikowanego fizjoterapeutę i/lub rehabilitanta, oraz wsparcie wspierające,realizowane w celu zwiększ.bezpieczeństwa osób objętych pomocą: -doposażenie gr. docelowej w sprzęt i środki higieniczne ułatwiające codzienne funkcjonowanie.
