The investment submitted for funding consists of the acquisition of fixed assets in the form of: Ultrasound with paediatric cover, ophthalmology unit and a set of devices for ophthalmology clinic, which will allow to increase the possibility of diagnostic ophthalmological and GP and gynaecological clinics to meet the growing needs of patients. The equipment will be used in NZOZ Biogenes – Pl. Bzowy 1, Wrocław. The purchased equipment will not be used for payment benefits. The main recipients of the project will be patients registered in GA and AOS NZOZ Biogenes, newly registered patients and patients cooperating with the Applicant of establishments. The main objective of the project is to increase the provision of integrated care service, which puts the patient at the centre of activities, and coordinates the next stages of treatment. At present, such solutions are rare in health care, while emphasis is placed on changing the way healthcare services are managed and offering services. The changes made will result in better management and provision of services to the patient they need. Coordination will be done by a family doctor, who will supervise sending the patient to other specialists. Services will provide a very high added value for the Applicant and partners, and will ensure a significant shift of the activities necessary for hospitalisation to the services offered under GE and AOS. The project aims to place the patient at the centre of all activities undertaken by providers and actions to ensure the comprehensiveness and continuity of care for the patient. The project is implemented in partnership. It shall demonstrate supraregional complementarity. The partnership plans, inter alia, actions to ensure the comprehensiveness and continuity of patient care, actions taken in an integrated manner, and the use of human resources of partners. (English)

    Przedstawiona do dofinansowania inwestycja polega na nabyciu środków trwałych w postaci: USG z nakładką pediatryczną, unitu okulistycznego oraz zestawu urządzeń dla poradni okulistycznej, które umożliwią zwiększenie możliwości diagnostycznej poradni okulistycznej oraz POZ oraz ginekologicznej co pozwoli na zaspokajanie rosnących potrzeb pacjentów. Sprzęt będzie wykorzystywany w placówce NZOZ Biogenes - Pl. Bzowy 1, Wrocław. Zakupywany sprzęt nie będzie wykorzystywany do świadczeń odpłatnych. Głównymi odbiorcami projektu będą pacjenci zarejestrowani w POZ oraz AOS NZOZ Biogenes, pacjenci nowo zarejestrowani oraz pacjenci współpracujących z Wnioskodawcą placówek. Głównym celem realizacji projektu jest wzrost świadczenia usługi opieki koordynowanej, która stawia pacjenta w centrum działań, oraz koordynuje kolejne jego etapy leczenia. Obecnie takie rozwiązania są rzadkie w opiece zdrowia, jednocześnie kładzie się nacisk na zmianę sposobu zarządzania i oferowania usług w ramach opieki zdrowotnej. Wprowadzone zmiany spowodują lepsze zarządzanie i oferowanie usług pacjentowi, których potrzebuje. Koordynacją zajmie się lekarz rodzinny, który będzie nadzorował wysyłanie pacjenta do kolejnych specjalistów. Usługi będą stanowić bardzo wysoką wartość dodaną dla Wnioskodawcy i partnerów, zapewnią również znaczne przesunięcie czynności koniecznych do przeprowadzenia w ramach hospitalizacji do usług oferowanych w ramach POZ i AOS. Projekt zakłada zrealizowanie celu umieszczenia pacjenta w centrum uwagi wszelkich działań podejmowanych przez świadczeniodawców oraz działania zapewniające kompleksowość i ciągłość opieki nad pacjentem. Projekt jest realizowany w partnerstwie. Wykazuje się komplementarnością ponadregionalną. W ramach partnerstwa planuje się m.in. działania zapewniające kompleksowość i ciągłość opieki nad pacjentem, działania podejmowane w sposób zintegrowany, korzystanie z zasobów ludzkich partnerów.
