The project aims at prevention and early diagnosis of symptoms of cerebrovascular diseases, in particular strokes and transient necrocytic attack (TIA), persons in the age of occupational activity(40-65years) in the period from 1.12.2017 to 31.10.2020, on the territory of the voivodships: Zachodniopomorskie, Wielkopolska, Kujawsko-Pomorskie and Pomorskie. The project foresees the support of two target groups: medical staff of entities providing GP services (cooperation with 60 GEs from the project area) and persons aged 40-65 from the risk of stroke.The project includes 3 tasks: educational activities for GEs, educational activities for patients and screening. The main result of the project is the implementation of a preventive programme in the field of diseases adversely affecting labour resources. 7800 people (including 3900 K and 3900 M) will benefit from a medical service in a preventive programme co-financed by the ESF, and 120 people (including 53K and 67M) cooperating or working for primary care facilities through the ESF will increase their competence in implementing ESF prevention programmes. Projekt ma na celu profilaktykę i wczesne rozpoznanie objawów chorób naczyń mózgowych, w szczególności udarów mózgu i przemijającego atakuniedokrwiennego(TIA),uosóbwwiekuaktywnościzawodowej(40-65lat)wokresie od 1.12.2017do 31.10.2020, naterenie województw:zachodniopomorskiego,wielkopolskiego,kujawsko-pomorskiego ipomorskiego. W ramach projektu przewiduje się wsparcie dwóch grup docelowych: personelu medycznego podmiotów świadczących usługi POZ (współpraca z 60 POZ z obszaru realizacji projektu) oraz osób w wieku 40-65 lat z grupy ryzyka wystąpienia udaru mózgu.Przedmiotowy projekt obejmuje realizację 3 zadań: działania edukacyjne dla POZ, działania edukacyjne dla pacjentów oraz badania przesiewowe. Głównym rezultatem projektu jest wdrożenie programu profilaktycznego w zakresie chorób negatywnie wpływających na zasoby pracy. 7800 osób (w tym 3900 K i 3900 M) skorzysta z usługi medycznej w programie profilaktycznym dofinansowanym w ramach EFS, a120 osób (w tym 53K i 67M) współpracujących lub pracujących na rzecz placówek podstawowej opieki zdrowotnej dzięki EFS podniesie swoje kompetencje z zakresu wdrażania programów profilaktycznych opracowanych ze środków EFS.
