Projekt "Wypracowanie i pilotażowe wdrożenie modelu kompleksowej rehabilitacji umożliwiającej podjęcie lub powrót do pracy" przewiduje wypracowanie i przetestowanie modelu kompleksowej rehabilitacji, ułatwiającej podjęcie lub powrót do aktywności społecznej lub zawodowej osób, które wskutek doznanego urazu, zdiagnozowanego schorzenia lub niepełnosprawności wrodzonej lub nabytej w okresie rozwojowym nie mogą kontynuować lub podejmować się pełnienia ról społecznych oraz kontynuować lub podejmować aktywności zawodowej. Model ten ma pozwolić na przygotowanie osoby, która w efekcie urazu lub zdiagnozowanego schorzenia albo niepełnosprawności wrodzonej lub nabytej w okresie rozwojowym jest zagrożona lub dotknięta długotrwałą albo trwałą utratą możliwości podejmowania ról społecznych oraz zatrudnienia, do powrotu do pełnienia ról społecznych i zawodowych lub ich podjęcia, w oparciu o analizę, ocenę i rozwój jej indywidualnego potencjału, cech psychofizycznych i posiadanych kompetencji osobistych i zawodowych z wykorzystaniem ICF.W ramach pilotażu modelu przewiduje się objęcie kompleksową rehabilitacją co najmniej 600 osób w czterech ośrodkach, które zostaną wyłonione w drodze przetargu.Projekt będzie realizowany we współpracy partnerskiej Państwowego Funduszu Osób Niepełnosprawnych jako Lidera projektu z Zakładem Ubezpieczeń Społecznych oraz Centralnym Instytutem Ochrony Pracy, który został wyłoniony w drodze konkursu zgodnie z art. 33 ustawy z dnia 11 lipca 2014 r. o zasadach realizacji programów w zakresie polityki spójności finansowanych w perspektywie finansowej 2014 2020.
