Celem głównym proj jest dokonanie postępu w procesie aktywizacji społeczno- zatrudnieniowej przez min. 34% spośród 58 osób (34K, 24 M), zagrożonych ubóstwem lub wykluczeniem społecznym, zamieszkujących w myśl KC woj. wielkopolskie, Gminę Białośliwie, poprzez udział w kompleksowym wsparciu w okresie od 01.06.2018 do 30.11.2019.W ramach projektu założono osiągnięcie następujących wskaźników:• Liczba osób zagrożonych ubóstwem lub wykluczeniem społecznym objętych wsparciem w programie – 58• Liczba osób z niepełnosprawnościami objętych wsparciem w programie - 15• Liczba osób zagrożonych ubóstwem lub wykluczeniem społecznym, które uzyskały kwalifikacje po opuszczeniu programu – 18• Liczba osób zagrożonych ubóstwem lub wykluczeniem społecznym poszukujących pracy po opuszczeniu programu – 20.• Liczba osób zagrożonych ubóstwem lub wykluczeniem społecznym pracujących po opuszczeniu programu (łącznie z pracującymi na własny rachunek) – 12. Grupę docelową stanowi 58 osób (34 K, 24 M) zamieszkałych w Gminie Białośliwie, zagrożonych ubóstwem lub wykluczeniem społ. Do proj zostaną przyjęte osoby spełniające min. 1 przesłankę zagrożenia ubóstwem lub wykluczeniem społecznym:- os bierne ZAW - 58 UP (34 K, 34 M),- os niepełnosprawne – 15 UP, w tym 10 o stopniu umiarkowanym- os korzystające z POPŻ - 18 os (9K, 9 M) - min. 30% UP.W ramach projektu zostaną zrealizowane takie zadania, jak:1. Aktywizacja społeczna2. Aktywizacja zdrowotna3. Aktywizacja edukacyjna4. Aktywizacja zawodowa5. Działania o charakterze środowiskowym. (Polish)

The principal objective is to make progress towards an employment and social activation process by at least 34 % of the 58 persons (34K, 24 M), at risk of poverty or social exclusion, resident in Wielkopolskie, in the White Valley, by participating in the comprehensive support in the period from 01.06.2018 to 30.11.2019.The project assumes the achievement of the following indicators:• Number of people at risk of poverty or social exclusion in the programme — 58• Number of persons with disabilities supported in the programme — 15• Number of persons at risk of poverty or social exclusion who have obtained a qualification after leaving the programme — 18• Number of people at risk of poverty or social exclusion who work after leaving the programme — 20.• Number of people at risk of poverty or social exclusion working after leaving the programme (including self-employment) — 12. The target group is 58 people (34 K, 24 M) who live in the Gmina Bialowieza, who are at risk of poverty or social exclusion. Persons meeting at least 1 are likely to be at risk of poverty or social exclusion: — passive circuits — 58 UP (34 K, 34 M), — s with a disability — 15 UP, including 10 degree of moderation, including degrees of moderation — 18 os (9K, 9 M) — min. 30 % UP.Within the project, will be carried out as follows: 1.
