Proj.zakłada stworzenie warunków na większą samodzielność i funkcjonowanie w najbliższym środowisku os. niesamodzielnych.Świadczenie długoterminowej op.medycznej w formie op.pielęgniarskiej odpowie na potrzeby os.niesamodzielnych dot.podstawowych potrzeb os.wymagających op.medycznej,ale niekwalifikujących się do leczenia szpitalnego.Odbiorcami usługi będą os.korzystające już ze świadczenia w ramach NFZ i wymagające zwiększenia liczby godz.wsparcia i os.nie korzystające jeszcze z NFZ a kwalifikujące się. Ścieżka interwencji w proj.obejmuje następujące etapy:rekrutacja przeprowadzona zgodnie z przygot.regulaminem naboru i uczestnictwa w proj.,weryfikacja oceny skali Barthel,w przypadkach koniecznych konsultacje lek.,reh.domowa, wsparcie psych.,logopedyczne os.niesamodzielnych i ich op.faktycznych jako ot.uczestnika proj. Grupa docelowa będzie obejmować również otoczenie os.niesamodzielnych(faktycznych i rodzinę os.niesamodzielnych). Działania skierowane dla os.niesamodzielnych poszerzone będą o wsparcie dla ich ot. – wsparcie psych. i szkoleniowe. Świadczone usługi będą bezpłatne–uczestnicy nie będą ponosić w związku z tym żadnych opłat. Proj.zakłada realizację usług które: wykraczają poza gwarantowane świadczenia op. zdrowotnej lub po wykazaniu że gwarantowana usługa zdrowotna nie będzie mogła zostać sfinansowana uczest. ze środków publ. w okresie trwania projektu lub te z usługi jednocześnie obejmować będą gwarantowane usługi i podstawowe stanowiące wartość dodaną do funkcjonującego systemu op. zdrowotnej np.dodatkowe godz.pielęgniarki,rehabilitacja domowa,logopeda,psycholog itp. Zaplanowane usługi realizowane będą uwzględniając indywidualne możliwości uczestnika i otoczenia – w dogodnych godzinach i terminach(popołudnia,wieczory,weekendy,itp.)Poprzez realizację projektu zostaną wsparte miejsca świadczenia usług zdrowotnych;otoczenie biorące udział w indywidualnym wsparciu psychologicznym i szkoleniu dla opiekunów w zakresie op.medycz nad os.niesam (Polish)

The intervention pathway consists of the following etapy:rekrutacja carried out in accordance with the procedure for selection and participation in proj., verification of the Barthel scale, in cases where the necessary consultations are needed. The target group will also include a dependent environment (de facto and dependent family). Actions aimed at the autonomous areas will be extended to support them. — psychomotor support and training. The services provided will be free of charge for all participants. Pro.To carry out the following services: they go beyond the guaranteed benefits of health and/or after demonstrating that the healthcare service cannot be financed from publ. for the duration of the project, or the service will also include guaranteed services and essential services providing added value to the functioning of the health system, including nurses, home rehabilitation, speech therapists, psychologists, etc. The planned services will take into account the individual capacity of the participant and the environment — at convenient times and timing (afternoon, evening, weekend, etc.) Through the implementation of the project, the site will be supported by the place of delivery of health services; the environment involved in individual psychological and training support for carers in the field of Domedics.
