Głównym celem projektu jest zwiększenie dostępności do usług opiekuńczych dla 90osób (75K,15M) na terenie Gminy Lubraniec (woj. kujawsko-pomorskie) w terminie 01.05.2019-30.04.2021, poprzez utworzenie 60 nowych miejsc świadczenia dziennych usług opiekuńczych w nowo powstałych Klubach Seniora oraz 1 miejsca świadczenia usług opiekuńczych w miejscu zamieszkania w ramach zatrudnienia 1 opiekuna. Cel ten przyczyni się do realizacji celu szczegółowego tj. Zwiększenie dostępności usług społecznych w szczególności usług środowiskowych, opiekuńczych oraz usług wsparcia rodziny i pieczy zastępczej dla osób zagrożonych ubóstwem lub wykluczeniem społecznym (Cel szczegółowy 3). Grupa docelowa: - 90 os.niesam., zagrożonych ubóstwem lub wykluczeniem społ. (75K) - osób niesamodzielnych, mieszk. w rozum. KC na terenie woj.kujawsko-pom.Zaplanowane zadania:1. Utworzenie i bieżące funkcjonowanie 15 miejsc świadczenia dziennych usług op.w formie Klubu Seniora w Siemnówku 2. Utworzenie i bieżące funkcjonowanie 15 miejsc świadczenia dziennych usług opiek. w formie Klubu Seniora w Ossowie3. Utworzenie i bieżące funkcjonowanie 30 miejsc świadczenia dziennych usług op. w formie Klubu Seniora w Lubrańcu 4. Zwiększenie liczby miejsc świad. usług opiek. w miejscu zamiesz. oraz zapewnienie teleopieki na terenie Gminy Lubraniec.5. Realizacja wyjazdów kulturalnych dla 40 seniorów z Klubów Seniora w Gminie Lubraniec. Główne założenie/efekty:-Utworzenie 60 nowych miejsc Klubie Seniora w Gminie Lubraniec.-Zwiększenia liczby miejsc świadczenia usług społecznych w społeczności lokalnej poprzez zatrudnienie 1 opiekuna oraz liczby osób objętych usługami świadczonymi w społeczności lokalnej (5 os. w tym 5 k) na terenie Gminy Lubraniec, - Wsparcie 5 (K) osób zagrożonych ubóstwem lub wykluczeniem społecznym w ramach dziennych usług opiekuńczych. -Wsparcie 30 osób w postaci teleopieki. The main objective of the project is to increase the availability of care services for 90-01.05.2019 persons (75K, 15M) in the municipality of Lubraiec (Kujawsko-Pomorskie Province) by the -30.04.2021 deadline, with the creation of 60 new care services for daily care services in the newly created Senior Citizens’ Clubs and 1 places of care services in the place of residence of 1 carers. This objective will contribute to the specific objective: Increase the availability of social services, in particular community-based services, care services and family support services and foster care services for people at risk of poverty or social exclusion (Specific objective 3). Audience: — 90 Non-male, at-risk of poverty or social exclusion (75k) — dependent persons living in separation. KC in war between Kujawsko-Pomorskie and the planned tasks: 1. The creation and running of the 15 daily service of the daily service of the Senior Citizens’ Club in Siemnówki 2. The establishment and current operation of 15 daily service facilities in the form of the Seniora Seniora Club of Osso3. The creation and day-to-day operation of the 30 days of the provision of daily services, in the form of a Senior Senior Club, in Ljubljana, 4. Increase in the number of services provided in the area of service in the place of residence and the provision of care services in the municipality of Lubranicc.5. Implementation of cultural missions for 40 senior citizens in the Senior Citizens’ Club in the municipality of Lubraiec. Main assumption/effect: — the creation of the 60 new places of the Senora Senora in the municipality of Lubraniec.-Increasing the number of social services in the local community through the employment of 1 caregivers and the number of people involved in the provision of services in the local community (5 of which 5 k) in the municipality of Lubraiec, — Support to 5 (K) people at risk of poverty or social exclusion, as part of daily care services. — Support for 30 people in the form of telecare.
