Projekt dotyczy przetestowania modelu środowiskowego wsparcia osób z zaburzeniami i chorobami psychicznymi, wypracowanego w konkursie I etapu. Celem głównym projektu będzie poprawa jakości działań w obszarze zdrowia psychicznego poprzez przetestowanie modelu wsparcia osób z zaburzeniami i chorobami psychicznymi wypracowanego w konkursie I etapu w powiecie lubelskim ziemskim i grodzkim do II 2021 r.Główne rezultaty:- wyposażenie użytkowników innowacji w kompleksowe narzędzia leczenia i terapii osób z podwójną diagnozą,- zwiększenie dostępności wśród odbiorców do zintegrowanego i skutecznego leczenia w wyspecjalizowanym ośrodku,- wyposażenie szerokiego grona użytkowników pośrednich innowacji w umiejętności pracy z osobami z podwójną diagnozą,- osiągnięcie efektu synergii terapii medycznej i rehabilitacji społecznej.Grupę docelową w wymiarze testowania stanowić będą: użytkownicy i odbiorcy innowacyjnego modelu.Odbiorcami pośrednimi będą członkowie rodzin pacjenta i członkowie jego najbliższego otoczenia Główne zadania: powołanie Centrum Zdrowia Psychicznego, wsparcie pacjentów z podwójną diagnozą psychiatryczną i ich rodzin w ramach CZP, szkolenie dla użytkowników pośrednich, weryfikacja i modyfikacja modelu uwzględniająca wyniki testowania i przekazanie jej do IOK.Oświadczamy, że projekt dotyczy przetestowania modelu środowiskowego wsparcia osób z zaburzeniami i chorobami psychicznymi, wypracowanego w konkursie I etapu. Jednakże posiadamy pisemną zgodę IOK na jego modyfikację w zakresie ograniczenia lokalizacji testowania i zmian w partnerstwie (pismo DZF.IX.7611.28.2017.ES.3 z dn.7.09.2017 r.) (Polish)

The project concerns the testing of an environmental model for the support of people with mental disorders and illnesses, which is the result of the Phase I competition. The main objective of the project will be to improve the quality of activities in the field of mental health, by testing a model of support for people with mental disorders and illnesses developed in the Phase I competition in the District of Lubelskie and terrestrial and god to II 2021. main results: — equipping the users of innovation with comprehensive treatment and treatment tools for people with double diagnosis, — improving access to integrated and effective treatment for people in a specialised centre, — equipping a wide audience of indirect innovators with dual diagnosis skills, — achieving synergy effects on medical treatment and social rehabilitation. the target group in the testing dimension will be: the users and the user of the innovative modem will be members of the patient’s family and the closest surroundings of the main tasks of: the establishment of the Centre for Mental Health, the support of patients with dual psychiatric diagnoses and their families in the context of the CZP, training for intermediate users, the verification and modification of a model which takes account of the results of the testing and its transmission to IOKNO.He is of the opinion that the project is concerned with testing the environmental model for the support of people with mental disorders and illnesses, which is the result of the Phase I competition.
