    Projekt se zaměřuje na nalezení a udržení stabilního bydlení u lidí se závažným duševním onemocněným ohrožených bezdomovectvím. Nejvíce je bezdomovectvím ohrožena ta část lidí z CS, která vypadává ze zdravotní a sociální péče. Tato část bude aktivně vyhledána a podpořena službou sociální rehabilitace. Vyhledání proběhne jak ve zdravotnických zařízeních, tak prostřednictvím spolupráce s majiteli bytů. Cílem je stabilizace bytové situace a zvýšení schopností pro zvládání samostatného života. (Czech)

    The project aims at finding and maintaining stable living for people with mental ill affected homelessness. The part of the CS that comes out of health and social care is at greatest risk of homelessness. This session will be actively sought and supported by a social rehabilitation service. The search will take place both in healthcare settings and in cooperation with landlords. The aim is to stabilise the housing situation and to increase the ability to deal with independent living.
