Grupę docelową stanowi 60 os.(36K,24M)osoby bezrobotne to jest 42 osoby i osoby bierne zawodowe to jest 18 osób zagrożonych ubóstwem lub wykluczeniem społecznym, doświadczających wielokrotnego wykluczenia społecznego rozumianego jako wykluczenie z powodu więcej niż 1 z przesłanek tj.: •osoby korzystające ze świadczeń z pomocy społecznej zgodnie z ustawą z dnia 12.03.2004r.o pomocy społecznej lub kwalifikujące się do korzystania ze świadczeń z pomocy społecznej to jest spełniające co najmniej jedną z przesłanek określonych w artykule 7 ustawy z dnia 12.03.2004r., •osoby korzystające z Programu Operacyjnego Pomoc Żywnościowa, •osoby zakwalifikowane do III profilu pomocy, zgodnie z ustawą z dnia 20.04.2004r.o promocji zatrudnienia i instytucjach rynku pracy, nie więcej niż 20% Uczestników Projektu to jest 12 osób, •osoby z niepełnosprawnością- minimum 30%Uczestników Projektu to jest 18 osób, zamieszkujących w rozumieniu Kodeksu Cywilnego na terenie woj.mazowieckiego w gminach poniżej progu defaworyzacji określonego w Mazowieckim barometrze ubóstwa i wykluczenia społecznego opisanych w pkt C.1.7 wniosku. Celem głównym projektu jest podwyższenie aktywności społeczno-zawodowej wyżej wymienionej grupy docelowej poprzez realizację zadań:(diagnozy indywidualnych potrzeb i potencjałów UP,poradnictwa psychologicznego, pośrednictwo pracy, treningu skutecznej komunikacji, warsztatów rozwoju osobistego,szkoleń i staży) od 01.08.2018r. do 31.07.2019r. prowadzącą do zatrudnienia minimum 18 osób oraz postępu w aktywizacji społecznej minimum 21 osób. W projekcie zostały opracowane następujące rezultaty: Liczba osób zagrożonych ubóstwem lub wykluczeniem społecznym, które uzyskały kwalifikacje po opuszczeniu programu-14 osób. -Liczba osób zagrożonych ubóstwem lub wykluczeniem społecznym poszukujących pracy po opuszczeniu programu-34 osoby. The target group is 60 (36K, 24M). the unemployed are 42 persons and 18 persons who are unemployed are persons at risk of poverty or social exclusion, who have multiple social exclusion as an exclusion on account of more than 1 conditions, i.e.: • persons benefiting from social assistance benefits in accordance with the Act of 12.03.2004 on social assistance or eligible for social assistance benefits are fulfilling one or more of the conditions laid down in Article 7 of the Act of 12.03.2004,• persons benefiting from the Operational Programme Food Aid,• persons qualified in the III Aid Profile, in accordance with the Act of 20.04.2004 on the promotion of employment and labour market institutions, of no more than 20 % of the Participants for the Project is 12 persons,• persons with a disability — a minimum of 30 % of the Participants to the Project is 18 persons, residing within the meaning of the Civil Code in the war of Mazowieckie in the municipalities below the threshold for disadvantage defined in the Mazowieckim trap of poverty and social exclusion as described in point C.1.7 of the proposal. The main objective of the project is to increase the socio-professional activity of the above-mentioned target group by fulfilling the tasks of: (diagnosis of individual needs and potential of UP, psychological counselling, job placement, effective communication, personal development workshops, training and internship workshops) from 01.08.2018 to 31.07.2019, with a minimum of 18 workers and a progress in social activation of at least 21 people. The following deliverables have been developed: The number of people at risk of poverty or social exclusion, which have obtained a qualification after leaving 14 people. — Number of people at risk of poverty or social exclusion looking for a job after leaving 34 people.
