Projekt realizowany przez NZOZ PRAXIS spółka z.o.o. skierowany jest do: 70 osób niesamodzielnych (w tym co najmniej 75% osób powyżej 65 r.życia) zamieszkujących na terenie miasta Nowy Sącz lub gminy Chełmiec,których stan zdrowia nie pozwala na pozostawanie wyłącznie pod opieką podstawowej opieki zdrowotnej i ambulatoryjnej opieki specjalistycznej, a jednocześnie nie wymagających całodobowego nadzoru lekarskiego i pielęgniarskiego realizowanego w trybie stacjonarnym spełniających m.in. następujące warunki: 1) a) osoby bezpośrednio po przebytej hospitalizacji, których stan zdrowia wymaga wzmożonej opieki pielęgniarskiej, nadzoru nad terapią farmakologiczną, kompleksowych działań usprawniających i przeciwdziałających postępującej niesamodzielności oraz wsparcia i edukacji zdrowotnej, a także doradztwa w zakresie organizacji opieki i leczenia osoby niesamodzielnej. b) osoby, którym w okresie ostatnich 12 miesięcy udzielone zostały świadczenia zdrowotne z zakresu leczenia szpitalnego. 2) osoby wymagające,ze wzgl.na stan zdrowia,świadczeń pielęgnacyjnych,opiekuńczych i rehabilitacyjnych oraz kontynuacji leczenia,a niewymagający hospitalizacji w oddziale szpitalnym,którzy w ocenie skalą poziomu samodzielności (skala Barthel) otrzymają 40-65 punktów. 3) osoby mające prawo do świadczeń opieki zdrow.finans.ze środków publicznych. Cel projektu to: Deinstytucjonalizacja opieki nad osobami zależnymi (w tym osobami powyżej 65 roku życia) z terenu miasta Nowego Sącza oraz gminy Chełmiec poprzez utworzenie i prowadzenie Dziennego Domu Opieki Medycznej (DDOM). W ramach proj. utworzony i doposażony zostanie DDOM (deinstytucjonalizacja opieki nad osobami zależnymi) z 12 miejscami, realizowane będą świadczenia zdrowotne dla ogółem 70 pacjentów,w tym 75% osób powyżej 65 r.życia, prowadzone będą szkolenia dla pacjentów i ich rodzin/ opiekunów. Założono, że min. 80% osób korzystających z usług DDOM będzie pozytywni (Polish)

The project implemented by NZOZ PRAXIS will be aimed at:70 dependants (including at least 75 % of the population aged 65 and over) who reside in Nowy Sącz and/or the municipality of Chełłmale, whose state of health does not allow care to be taken only in the care of primary care and outpatient care, but not requiring round-the-clock medical supervision and nursing care which meets, inter alia, the following conditions:(1) (a) persons directly following the rate of hospitalisation whose state of health requires increased nursing care, surveillance of pharmacological treatment, comprehensive action to facilitate and counter the continuing dependence and health support and education, as well as advice on the organisation of the care and treatment of the person not independent (b) the person to whom health care benefits have been provided in respect of hospital treatment during the last 12 months.2) persons requiring, for reasons of health, care, care and rehabilitation and continuation of treatment, and not requiring hospitalisation in a hospital unit, who will receive between 40 and 65 points in the assessment of the degree of autonomy (Barthel scale).3) persons entitled to health care benefits.The aim of the project is to:Deinstitutionalisation of care for dependent persons (including those over 65) from the town of Nowy Sącz and the municipality of Chełm, through the creation and running of the Daily Care Service (DDOM).In the framework of the creation of a DDOM (deinstitutionalisation of care for dependents) with 12 places, health services will be provided for a total of 70 patients, including 75 % of those over 65, training for patients and their families/carers will be provided.It has been assumed that at least 80 % of the beneficiaries of the DDOM services will be positive (English)
