Przedmiotem projektu jest termomodernizacja budynku przychodni zdrowia przy ul. Wajdy 10 w Bartoszycach. W ramach przedsięwzięcia termomodernizacyjnego zaplanowano do wykonania następujące prace stanowiące koszt kwalifikowany projektu: 1. Modernizacja istniejącej instalacji centralnego ogrzewania. 2. Wymiana okien zewnętrznych, a następnie montaż okien z profili PVC. Pozostałe otwory okienne należy zamurować. 3. Zdemontowanie istniejących starych drzwi zewnętrznych i zamontowanie nowych drzwi zewnętrznych. Pozostałe otwory drzwiowe należy zamurować. W ramach przedsięwzięcia przewiduje się montaż wiatrołapu w drzwiach wejściowych. 4. Wykonanie ocieplenia ścian zewnętrznych z użyciem styropianu. 5. Wykonanie ocieplenia ścian zewnętrznych piwnicy (cokołowych) oraz ścian piwnic przy gruncie z użyciem styropianu. Zakres projektu obejmuje także wykonanie towarzyszących prac stanowiących koszty niekwalifikowane: 1. Rozbiórka zadaszenia. Projekt będzie wdrażany przez Urząd Miasta Bartoszyce w zakresie kompetencji Wydziału Techniczno-Inwestycyjnego. Do najważniejszych etapów realizacji projektu należy zaliczyć: - wykonanie zadań inwestycyjnych jako ww. koszty kwalifikowane (zadania 2, 3, 4, 5) w terminie od III kw. 2014 do IV kw. 2014 - wyłonienie wykonawcy w procedurze Prawa zamówień publicznych jako ww. koszty kwalifikowane (zadanie 1) – założono podpisanie umowy z wykonawcą w IV kw. 2017 r. - właściwą realizację rzeczową i finansową jako ww. koszty kwalifikowane (zadanie 1) w terminie od IV kw. 2017 do I kw. 2019 Projekt wpisuje się w założenia określone SzOOP RPO WiM realizując cel Poddziałania 4.3.1 określony jako Zwiększona efektywność energetyczna budynków użyteczności publicznej. As part of the thermomodernisation project, the following works are planned to be carried out at the cost of the eligible project: 1. The upgrade of the existing central heating system. 2. Replacement of outer windows and the subsequent installation of windows in PVC. The other window openings must be filled in. 3. The removal of existing old external doors and the installation of new external doors. The remaining door openings shall be filled in. The project provides for the installation of a rostrum in the entrance door. 4. The insulation of external walls by the use of polystyrene. 5. The insulation of the external walls of the basement and the basement of the cellar walls on the ground using the polystyrene. The scope of the project shall also include execution of work accompanying non-eligible costs: 1. Demolition. The project will be implemented by Bartoszyce City Office within the remit of the Technical and Investment Service. The key steps in the implementation of the project include: — performance of the investment tasks as above eligible costs (tasks 2, 3, 4, 5) within the period from third quarter of 2014 to 4th quarter 2014 — Appointment of an economic operator in the procedure for the award of public contracts as the above eligible costs (task 1) — it is assumed that the contract will be signed with a contractor in the fourth quarter of the year. 2017 — appropriate material and financial implementation as above eligible costs (task 1) within the deadline of 4th quarter 2017 to Q1 2019 The project is in line with the objectives set by the ROP WiM in pursuing the objective of Sub-measure 4.3.1, which has been identified as a major energy efficiency in public buildings. Conformity to the type of project is specified in section 3.7 of the proposal. (English)
