Projekt (typ 1) jest skierowany do 50 osób (37K) zagrożonych ubóstwem i wykluczeniem społecznym, potrzebujących wsparcia w codziennym funkcjonowaniu, w tym niepełnosprawnych, zamieszkałych na terenie woj. zach-pom w Gminie Pełczyce oraz ich 10 opiekunów faktycznych (7K). Celem proj jest zwiększenie dostępności do usług opiekuńczych dla 50 os potrzebujących wsparcia w codziennym funkcjonowaniu (dalej: OPW, 37K), poprzez m.in. stworzenie 43 nowych miejsc świadczenia usług opiekuńczych oraz zwiększenie kompetencji 10 opiekunów fakt.(7K) z zakresie opieki nad podopiecznymi w okr. 01.07.2022- 31.10.2023. Proj odpowiada na problemy i potrzeby w świadczeniu usług społ., zidentyfikowane na obszarze jego real. biorąc pod uwagę trendy demograf. i poziom dostępności usług społ. na tym obszarze. Proj będzie skierowany do os potrzeb. wsparcia w codziennym funkcjonowaniu (dalej: OPW), w tym niepełnos. Zadania w ramach proj: usługi opiekuńcze, opiekuńcze specjal., wsparcie opiekunów faktycznych, Klub Seniora, co przyczyni się do zwiększenia autonomii UP, zwiększenie dostępności i bezpieczeństwa dla podopiecznych. Usługi będą świadczone w oparciu o wysokie standardy przez specjalistyczną kadrę, przygotowaną do świadczenia tych usług, w większości w miejscach zamieszkania. Usługi te będą służyć zaspokojeniu rosnących potrzeb wynikających z niesamodzielności. Dzięki proj 50 OPW (37K) będzie mogło otrzymać niezbędne wsparcie poza systemem instytucjonalizowanej pomocy, przez co dłużej będą mogły być aktywne w społeczności lokalnej. Rezultaty: 1. Liczba miejsc świadczenia usług społ. istniejących po zakończeniu projektu: 43 (1 opiekunka, 1 rehabilitant, 1 psycholog, 40 miejsc w Klubie Seniora) 2. Liczba osób zagrożonych ubóstwem lub wykluczeniem społecznym objętych usługami społecznymi świadczonymi w programie - 50. (Polish)

The project (type 1) is aimed at 50 people (37K) at risk of poverty and social exclusion, who need support in their daily functioning, including the disabled, living in the Zach-pom province in the Municipality of Pełczyka and their 10 actual carers (7K). The aim of the project is to increase access to care services for 50 people in need of support in their daily functioning (hereinafter: GIP, 37K), through, inter alia, the creation of 43 new places for the provision of care services and increasing the competences of 10 caregivers fact.(7K) in the field of care for children in the field of care in the field of care for children. 01.07.2022-31.10.2023. Project responds to problems and needs in the provision of social services, identified in its real area. taking into account demographic trends and the level of availability of social services in this area. The project will be addressed to the needs of support in everyday functioning (hereinafter: GW), including disability. Tasks under the project: care services, care specialty., support of actual caregivers, Senior Club, which will contribute to increasing the autonomy of UP, increasing accessibility and safety for pupils. The services will be provided on the basis of high standards by specialist staff prepared to provide these services, for the most part in places of residence. These services will serve to meet growing needs arising from dependency. Project 50 GIPs (37K) will be able to receive the necessary support outside the system of institutionalised assistance, which will allow them to be active longer in the local community. Results: 1. Number of places to provide social services existing at the end of the project: 43 (1 babysitter, 1 therapist, 1 psychologist, 40 places in the Senior Club) 2.
