The main objective (CG) of the project (P) is to increase access to care services for 36 GIPWCFs (25K and 11M) after 60 years of age from the area of Pow.bieszczadzki by providing mobile home care services for 6 GIPWCF (3K and 3M) after 60 years of age and continuing support under the DDP in Ustrzyki Dolne for 30 GIPWCF (22K and 8M) after 60 years of age from the area of Bieszczady County in the period 01.03.2021-31.08.2023 R. P. will contribute to the specific objective of the ROP WP, which is to increase the availability of social services, in particular environmental and care services for people at risk of poverty or to exclude the community by: 1.Provision of mobile home care ● implementation of the forms of support provided for in tripartite contracts, i.e. services provided at the place of residence to the GIPWCF — Project Participants (OP): ● care services provided by day caregivers (e.g. environmental caregiver, nurse)The above-mentioned activities aim to organise the care of the GIPWCF for as long as possible in the place of residence and postponement during the necessity of 24-hour institutional care for the GIPWWCF. Classes will be individualised, taking into account the type and degree of disability (NSPR.) and the expectations of UP. As a result of the implementation of the actions, project: ● 10 GIPWCF will receive support in the form of care services, which will translate into an increase in their standard of living. 2.Continuing support in the DDP, which will be provided by UP as part of its mission: ● meeting life needs, inter alia by providing a place for safe and dignified spending of time ● care and care services, including assistance in personal hygiene, ● two meals a day ● enabling participation in therapeutic, artistic, musical and movement-supporting activities ● access to books and media, modern technologies, ● social activation, organisation of cultural, recreational and social events ● assistance in the development and strengthening of activities and psychological assistance (English) | |||

Celem głównym (CG) projektu (P) jest zwiększenie dostępności do usług opiekuńczych dla 36 OPWWCF (25K i 11M) po 60 roku życia z terenu pow.bieszczadzkiego poprzez świadczenie usług mobilnej opieki domowej dla 6 OPWWCF (3K i 3M) po 60 roku życia oraz kontynuowanie wsparcia w ramach DDP w Ustrzykach Dolnych dla 30 OPWWCF (22K i 8M) po 60 roku życia z terenu powiatu bieszczadzkiego w okresie 01.03.2021 - 31.08.2023 r. P. przyczyni się do realizacji celu szczegółowego RPO WP jakim jest zwiększenie dostępności usług społ. w szczególności usług środowiskowych i opiekuńczychdla os. zagrożonych ubóstwem lub wyklucz. społ. poprzez: 1.Świadczenie mobilnej opieki domowej ● realizację form wsparcia przewidzianych w kontraktach trójstronnych, tj. usług świadczonych w miejscu zamieszkania na rzecz OPWWCF - Uczestników projektu (UP): ● usługi opiekuńcze świadczone przez Opiekunów dziennych (np. opiekun środowiskowy, pielęgniarka)Ww. działania mają na celu organizację opieki nad OPWWCF jak najdłużej w miejcu zamieszkania i odraczanie w czasie konieczności całodobowej opiekiinstytucjonalnej nad OPWWCF. Zajęcia będą zindywidualizowane, uwzględniające rodzaj i stopień niepełnosprawności (nspr.) oraz oczekiwania UP. W rezultacie realizacji działań proj.: ● 10 OPWWCF otrzyma wsparcie w postaci usług opiekuńczych, co przełoży się na wzrost ich standardu życia. 2.Kontynuowanie wsparcia w DDP, które w ramach swojej działności zapewni UP: ● zaspokojenie potrzeb życiowych, m.in. poprzez zapewnienie miejsca do bezpiecznego i godnego spędzania czasu ● usługi opiekuńcze i pielęgnacyjne, w tym pomoc w utrzymaniu higieny osobistej, ● dwa posiłki dziennie ● umożliwienie udziału w zajęciach terapeutycznych, plastycznych, muzycznych i wspierających ruchowo ● dostęp do książek i środków przekazu, nowoczesnych technologii, ● społeczną aktywizację,organizowanie imprez kulturalnych, rekreacyjnych i towarzyskich ● pomoc w rozwinięciu i wzmacnianiu aktywności oraz samodzielno ● pomoc psychologiczną (Polish)
