The aim of the project is the social and professional reintegration of 60 (including 36k) people at risk of poverty or social exclusion, including in particular people with disabilities, in the first place requiring social activation, from the municipality of Krynica-Zdrój, which will take place by mid 2022. Target group: 60 persons (including at least 60 % of women and 10 % of persons with disabilities) or their families at risk of poverty or social exclusion, in accordance with Article 33 of the rules of the competition, from the municipality of Krynica-Zdrój (in accordance with the CC). The main tasks: 1) Identification of individual needs and diagnosis of opportunities of participants 2) Individual employment mediation 3) Psychological and psychosocial counselling 4) Training of social competences and skills 5) Training 6) Internships and support of a supported employment trainer 7) Social work. Main results: 1) l. persons at risk of poverty or social exclusion supported by the programme — 60 persons. 2) l. persons with disabilities supported in the program — 6 persons. 3) l. persons at risk of poverty or social exclusion who have obtained qualifications after leaving the programme — 39 persons. 4) l. persons at risk of poverty or social exclusion seeking employment after leaving the programme — 21 persons. 5) l. persons at risk of poverty or social exclusion working after leaving the scheme (including self-employed persons) — 15 persons. 6) social and professional efficiency at least at the level specified in the rules of the competition (English) | |||

Celem projektu jest reintegracja społeczna i zawodowa 60 (w tym 36k) osób zagrożonych ubóstwem lub wykluczeniem społecznym, w tym w szczególności osób z niepełnosprawnościami, w pierwszej kolejności wymagających aktywizacji społecznej, z gminy Krynica-Zdrój, co nastąpi do połowy 2022r. Grupa docelowa: 60 osób (w tym co najmniej 60% kobiet oraz 10% osób z niepełnosprawnościami) lub ich rodziny zagrożone ubóstwem lub wykluczeniem społecznym, zgodnie z art. 33 regulaminu konkursu, z gminy Krynica-Zdrój (zam. zgodnie z KC). Główne zadania: 1) Identyfikacja indywidualnych potrzeb i diagnoza możliwości uczestników 2) Indywidualne pośrednictwo pracy 3) Poradnictwo psychologiczne i psychospołeczne 4) Trening kompetencji i umiejętności społecznych 5) Szkolenia 6) Staże i wsparcie trenera zatrudnienia wspieranego 7) Praca socjalna. Główne rezultaty: 1) l. osób zagrożonych ubóstwem lub wykluczeniem społecznym objętych wsparciem w programie - 60 os. 2) l. osób z niepełnosprawnościami objętych wsparciem w programie - 6 os. 3) l. osób zagrożonych ubóstwem lub wykluczeniem społecznym, które uzyskały kwalifikacje po opuszczeniu programu - 39 os. 4) l. osób zagrożonych ubóstwem lub wykluczeniem społecznym poszukujących pracy po opuszczeniu programu - 21 os. 5) l. osób zagrożonych ubóstwem lub wykluczeniem społecznym pracujących po opuszczeniu programu (łącznie z pracującymi na własny rachunek) - 15 os. 6) efektywność społeczna i zawodowa co najmniej na min. poziomie określonym w regulaminie konkursu (Polish)
