Main objective: Increased accessibility to high-quality social services for 20 people (15K, 5M) 50 years, at risk of poverty and social exclusion living in the municipality of Bulkowo (Płock district) who need support in everyday functioning by supporting the creation/functioning of an institution providing social services in the local community in the form of a Senior Club until 31.12.2022. The project is addressed to the target group from the area of Mazowieckie Voivodeship, Płock district (in the provincial report of natural persons they live in the area of the province of Mazowieckie — Płock district, Bulkowo commune in the distribution of the Civil Code/KF). The project plans to set up, equip and operate for 24 months a senior club, implementation of support in accordance with the individual needs of the UP, Support for people in need of support in everyday functioning implemented under the project will be based on the “General European Guidelines for the transition from institutional care to care provided at local community level”, Guidelines for the implementation of projects in the field of social inclusion and combating poverty using ESF and ERDF funds for the period 2014-2020 and in accordance with the minimum requirements for the provision of social services in the local community set out in the annex to the rules of the competition. RESULTS: The number of people at risk of poverty or social exclusion covered by social services of general interest in the programme; 20 o'cs. Cel główny: Wzrost dostępności do wysokiej jakości usług społecznych dla 20 osób (15K, 5M) pow. 50 r. ż., zagrożonych ubóstwem i wykluczeniem społecznym zamieszkałych na terenie gminy Bulkowo (powiat płocki) potrzebujących wsparcia w codziennym funkcjonowaniu poprzez wsparcie dla tworzenia/ funkcjonowania placówki świadczącej usługi społeczne w społeczności lokalnej w formie Klubu Seniora do 31.12.2022r. Projekt jest skierowany do grupy docelowej z obszaru woj. mazowieciego, powiat płocki (w przyp osób fizycznych zamieszkują one na obszarze woj. maz.- powiat płocki, gmina Bulkowo w roz.przep Kodeksu Cywilnego/KF). W projekcie planuje się utworzenie, wyposażenie i funkcjonowanie przez 24 miesiące klubu seniora , realizację wsparcia zgodnie z Indywidualnym zapotrzebowaniem UP, Wsparcie dla osób potrzebujących wsparcia w codziennym funkcjonowaniu realizowane w ramach projektu odbywać się będzie w oparciu o „Ogólnoeuropejskie wytyczne dotyczące przejścia od opieki instytucjonalnej do opieki świadczonej na poziomie lokalnych społeczności”, Wytyczne w zakresie realizacji przedsięwzięć w obszarze włączenia społecznego i zwalczania ubóstwa z wykorzystaniem środków EFS i EFRR na lata 2014-2020 oraz zgodnie z minimalnymi wymaganiami świadczenia usług społecznych w społeczności lokalnej określonymi w załączniku do regulaminu konkursu. REZULTATY: Liczba osób zagrożonych ubóstwem lub wykluczeniem społecznym objętych usługami społecznymi świadczonymi w interesie ogólnym w programie; 20 os.
