The subject of the project is the adaptation of the premises of the Department of National Rehabilitation and the Physiotherapy Department to the applicable regulations and needs, together with the purchase of medical equipment and other equipment for the above-mentioned branch and the physiotherapy department, in the SP ZOZ in Radziejowo.The investment covers the execution of construction works and the purchase of equipment necessary to increase the availability and quality of the provision of health services by the Applicant.The scope of the project includes:1. Application documentation.2. Construction work.3. Investor supervision.4. Equipment 8 %.5. Equipment 23 %.6. Promotion of the project.The investment will be implemented in the years 2021-2023. The planned timetable for the implementation of the project is realistic and takes into account the time necessary for the implementation of tender procedures, the execution of construction works and the supply and installation of medical equipment.The total investment outlays of the project amount to PLN 2 514 297.64 gross. The eligible costs of the investment amount to PLN 2 513 190.64 gross, while ineligible costs amount to 1 107,00 PLN gross, which result from the inability to qualify expenditure related to the IWOISZ application and the application for co-financing.The project will not be implemented in partnership. Przedmiotem projektu jest dostosowanie pomieszczeń Oddziału Rehabilitacji Ogólnoustrojowej oraz Działu Fizjoterapii do obowiązujących przepisów i potrzeb, wraz z zakupem sprzętu medycznego i pozostałego wyposażenia dla ww. oddziału oraz działu fizjoterapii, w SP ZOZ w Radziejowie.Inwestycja obejmuje swoim zakresem wykonanie robót budowalnych oraz dokonanie zakupu wyposażenia niezbędnego do zwiększenia dostępności i jakości świadczenia usług zdrowotnych przez Wnioskodawcę.Zakres projektu obejmuje:1. Dokumentację aplikacyjną.2. Roboty budowlane.3. Nadzór inwestorski.4. Wyposażenie 8%.5. Wyposażenie 23%.6. Promocję projektu.Inwestycja będzie realizowana w latach 2021-2023. Zaplanowany harmonogram realizacji projektu jest realistyczny i uwzględnia czas niezbędny na realizację procedur przetargowych, przeprowadzenie robót budowlanych oraz dostawę i instalację sprzętu medycznego.Całkowite nakłady inwestycyjne projektu wynoszą 2 514 297,64 zł brutto. Koszty kwalifikowane inwestycji wynoszą 2 513 190,64 zł brutto, natomiast koszty niekwalifikowane wynoszą 1 107,00 zł brutto, które wynikają z braku możliwości kwalifikowania wydatków związanych ze wnioskiem IWOISZ oraz wnioskiem o dofinansowanie.Projekt nie będzie realizowany w partnerstwie.
