The aim of the project is the deinstitutionalisation of health and social services in the communes of Buk and Stęszew through the creation of SCZP for adults and children and adolescents, which will support 250 (130K 120M) people, including 200 (100 K and 100 M) at risk of social exclusion or excluded from mental disorders in the period from 01.08.2021 to 30.06.2023. Support will be provided for another 50 people (20K 10M from the environment of children and 10K 10M from the adult environment) of people with mental disorders and 40 people in episodic mental health crises will be achieved through the implementation of the Psychological Health Centre for children through the creation of an integrated model of the mental health of the children through the creation of an integrated mental health centre for children and 40 people in episodic mental health crises and through the implementation of the Psychological Health Centre for children through the creation of an integrated model of the mental health of the children through the creation of an integrated mental health centre for children through the creation of an integrated mental health centre for children and 40 people in episodic mental health crises will be achieved through the implementation of the model of the mental health and mental health of the children through the creation of an integrated model of the mental health of the children through the creation of an integrated model of the mental health and mental health of the children. The solutions proposed in the project will allow for more effective support for people with mental disorders in the environment of their lives.The basis for integration and cooperation between health and social assistance sectors, as well as tailoring aid activities (direction and scope) to the individual needs of the patient by initiating and implementing innovative solutions among entities and professionals carrying out tasks for the target group.The deinstitutionalisation process will be carried out taking into account the needs of the individual target groups concerned by deinstitutionalisation and taking into account demographic trends.The project includes actions aimed at transferring healthcare services from the level of hospital treatment to primary health care and outpatient specialist care, mainly through: ensuring or strengthening the coordination of patient care and the development of de-institutionalised forms of patient care.     Celem projektu jest deinstytucjonalizacja usług zdrowotnych oraz społecznych na terenie gmin Buk i Stęszew poprzez utworzenie ŚCZP dla dorosłych oraz ŚCZP dzieci i młodzieży, co umożliwi wsparcie 250 (130K 120M) osób, w tym 200 (100 K i 100 M) zagrożonych wykluczeniem społecznym lub wykluczonych z tytułu zaburzenia psychicznego w okresie od 01.08.2021 r. do 30.06.2023 r. Wsparciem zostanie objętych jeszcze 50 os (20K 10M z otoczenia dzieci oraz 10K 10M z otoczenia dorosłych) osób z otoczenia osób z zaburzeniami psychicznymi oraz 40 osób w epizodycznych kryzysach zdrowia psychicznego Cel zostanie osiągnięty poprzez wdrożenie modelu zintegrowanej opieki środowiskowej na rzecz osób z zaburzeniami psychicznymi poprzez utworzenie Środowiskowego Centrum Zdrowia Psychicznego (deistytucjonalizacja ) dla dzieci i młodzieży oraz Środowiskowego Centrum Zdrowia Psychicznego dla dorosłych. Proponowane w projekcie rozwiązania umożliwią efektywniejsze wsparcie dla osób chorujących na zaburzenia psychiczne w środowisku ich życia.U podstaw leży integracja i współpraca między sektorami zdrowia i pomocy społecznej, oraz dopasowanie działań pomocowych (kierunek i zakres) do indywidualnych potrzeb pacjenta poprzez inicjowanie i wdrażanie nowatorskich rozwiązań wśród podmiotów i specjalistów realizujących zadania na rzecz grupy docelowej.Proces deinstytucjonalizacji będzie prowadzony z uwzględnieniem potrzeb poszczególnych grup docelowych, których dotyczy deinstytucjonalizacja oraz z uwzględnieniem trendów demograficznych.Projekt zawiera działania ukierunkowane na przeniesienie świadczeń opieki zdrowotnej z poziomu lecznictwa szpitalnego na rzecz podstawowej opieki zdrowotnej i ambulatoryjnej opieki specjalistycznej, przede wszystkim poprzez: zapewnienie lub wzmocnienie koordynacji opieki nad pacjentem i rozwój zdeinstytucjonalizowanych form opieki nad pacjentem.
