Proj.realizowany w partnerstwie: PLACÓWKA KSZTAŁCENIA USTAWICZNEGO EURO-CONSULTING i GOPS w ChodzieżyOkres realizacji: 01.06.2017-31.12.2018GŁÓWNYM CELEM jest aktywizacja społ.-zawodowa do 31.12.2018 r. grupy docelowej [=GD] 150 os.(83K/67M) zagrożonych ubóstwem lub wykluczeniem społ. , zamieszkujących w rozumieniu KC w woj. wielkopolskim [=WWLKP] w gminach Chodzież (wiejska), Chodzież (miejska), Margonin, Szamocin, Łobżenica, Miasteczko Krajeńskie, Wyrzysk, Okonek, Tarnówka, Jastrowie,w tym:-150 osób bez pracy: 90 (50K/40M) bezrobotnych (=BB) należących do III profilu pomocy oraz 60 (33K/27M) biernych zawodowo (=BZ) -15 osób (9K/6M) o umiarkowanym stopniu niepełnosprawności [=ON] -30 osób (17K/13M) spełniających przesłankę ubóstwa, dośw. wielokrotnego wykluczenia społ., rozumianego jako wykluczenie z powodu więcej niż jednej przesłanki-105 os. (58K/47M) korzystających z PO PŻ (przy czym zakres wsparcia dla tych os. nie powiela działań, które dana os. otrzymała lub otrzymuje z PO PŻ w ramach działań towarzyszących,o których mowa w PO PŻ).Osiągnięcie celu zapewnią kompleksowe działania zaplanowane do wdrożenia w ramach proj.:- diagnoza potrzeb i potencjałów UP wraz z utworzeniem IPD oraz podpisaniem kontraktu socjalnego - 150 UP,- specjalistyczne poradnictwo psychologiczne oraz prawne i obywatelskie - 150 UP,- treningi kompetencji i umiejętności społecznych - 150 UP,- szkolenia dla 110 UP- staże dla 120 UP.Dzięki realizacji projektu osiągnięte zostaną wskaźniki efektywności społ.-zatrudnieniowej na poziomie min. 56% (w tym efektywność zatrudnieniowa na poziomie min. 22%), a dla osób niepełnosprawnych w stopniu znacznym, intelektualnie, z niepełnosprawnościami sprzężonymi na poziomie min. 46% (w tym efektywności zatrudnieniowej na poziomie min. 12%)Ponadto osiągnięte zostaną wskaźniki rezultatu bezpośredniego: do 4 tyg. od zakończenia udziału w proj. 31% UP uzyska kwalifikacje, 32% będzie poszukiwało pracy, a 20% podejmie zatrudnienie (w tym na własny rachunek). (58K/47M) bénéficiant du PO (lorsque l’étendue du soutien à ces personnes ne fait pas double emploi avec les activités qu’une personne donnée a reçues ou reçoit du PO dans le cadre des mesures d’accompagnement visées dans le PO).La réalisation de l’objectif sera assurée par des actions globales prévues pour la mise en œuvre du projet:- diagnostic des besoins et des potentiels de l’OLP, création d’un DPI et signature d’un contrat social — 150 UP,- orientation psychologique, juridique et civique spécialisée — 150 UP,- formation des compétences et des compétences sociales — 150 UP,- formation pour 110 UP- stages pour 120 UP.
