The main objective of the project is to make progress in the process of social activation by at least 34 % of the 50 people at risk of poverty or social exclusion. (21 K, 29M) — wasps residing in the Fr. Jan Ziei in Cologne Ossa (hereinafter: MOW) through participation in the above-mentioned project from 01.05.2021 to 31.07.2022 in comprehensive project support. (e.g. social, health and professional activation). The target group is from Mazowieckie Voivodeship. (physicians who learn, work or reside in the province of maz. within the meaning of the Civil Code), i.e.: 50 people (21 K, 29M), at risk of poverty or social exclusion, inactive. The project responds to the problem of low access to social-professional activation activities and an insufficient level of socio-economic activity for pupils (hereinafter EXPENDED) MOW. Tasks in Project 1. ACTIVATION OF THE COMMUNITY. — development of turkey paths of reintegration, signing contracts on the model of a social contract, project type. 1.a., 1e. 2. Activation SP.- social, therapeutic, psychological, training, type project 1.e 3. Health activating UP — addiction therapy, rehabilitation support, project type 1.e 4. Activating UP — training, project type 1.e 5. ACTIVATION OF AW. Up — traineeships, project type 1.e 6. ACTIVATION OF AW. — counselling of the above mentioned tasks will contribute to the achievement of results and results, among others the number of people at risk of poverty or excluding the community: supported by the program.: 50 — who were qualified./acquired. after leaving.progr.: 38. Celem głównym proj jest dokonanie postępu w procesie aktywizacji społecznej przez min. 34% spośród 50 os zagrożonych ubóstwem lub wykluczeniem społ. (21 K, 29M)– os przebywających w Młodzieżowym Ośrodku Wychowawczym im. ks. Jana Ziei w Kolonii Ossa (dalej: MOW) poprzez udział w/w w okresie realiz. proj od 01.05.2021 do 31.07.2022 w kompleksowym wsparciu proj. (m.in. aktywizacja społeczna, zdrowotna, zawodowa). Grupa docelowa to os. z woj. mazowieckiego. (os. fiz., które uczą się, pracują lub zamieszkują w woj. maz. w rozumieniu Kodeksu Cywilnego), tj: 50 os (21 K, 29M), zagrożonych ubóstwem lub wykluczeniem społ., biernych zawodowo. Proj odpowiada na problem małego dostępu do działań w zakresie aktywizacji społ.-zawodowej oraz niewystarczający poziom aktywności społeczno- zawodowej dla wychowanków (dalej WYCH) MOW. Zadania w proj 1. AKTYWIZACJA SPOŁ. – opracowanie indyw. ścieżek reintegracji, podpisanie umów na wzór kontraktu socjalnego, typ proj. 1.a., 1e. 2. AKTYWIZACJA SP.- zaj. społeczne, terapeutyczne, psychologiczne, treningi, typ proj 1.e 3. AKTYWIZACJA ZDROWOTNA UP – terapia uzależnień, wsparcie rehabilitacyjne, typ proj 1.e 4. AKTYWIZACJA ZAWODOWA UP - szkolenia, typ proj 1.e 5. AKTYWIZACJA ZAW. UP - staże, typ proj 1.e 6.
