The aim of the project “Care services for the inhabitants of ŁOM” proposed by Caritas Archdiocese of Lodz is to improve access to implemented social services reducing poverty and social exclusion for 10/7/3 inhabitants of the ŁOM area aged over 18 years old until the end of 06.2023. The support will be carried out on the basis of an individualised support path, including an individual assessment of the material and life situation of a person in need of support in their daily functioning. The project will be comprehensive. Regular support in the form of care services, meetings with a physiotherapist (if necessary), regular access to the consultation of a social worker, access to rehabilitation equipment and training support for the care environment will be organised and implemented for UPs. The actions will contribute to the specific objective of ROP ILO2014-20. (English) | |||||||||||||||

Celem projektu "Usługi opiekuńcze dla mieszkańców ŁOM" proponowanego przez Caritas Archidiecezji Łódzkiej jestpoprawa dostępu do realizowanych usług społecznych ograniczających ubóstwo i wykluczenie społeczne dla 10/7/3mieszkańców obszaru ŁOM w wieku powyżej 18 r.ż. do końca 06.2023. Wsparcie będzie realizowane na postawieindywidualnie stworzonej ścieżki wsparcia, obejmującej również indywidualną ocenę sytuacji materialnej i życiowej danejosoby potrzebującej wsparcia w codziennym funkcjonowaniu. Projekt będzie kompleksowy. Dla UP będą zorganizowane iprowadzone systematyczne wsparcia w postaci usług opiekuńczych, spotkań z fizjoterapeutą (jeśli zajdzie taka potrzeba),regularny dostęp do konsultacji pracownika socjalnego, dostęp do sprzętu rehabilitacyjnego oraz wsparcie szkoleniowedla otoczenia osób objętych usługami opiekuńczymi. Działania przyczynią się do osiągnięcia celu szczegółowego RPO WŁ2014-20. (Polish)
