Целта на проекта е подобряване на качеството на живот и възможностите за социално включване на хората с увреждания и възрастните хора от Община Ивайловград, чрез осигуряване на услуги в домашна среда. Интегрираната услуга предвижда мобилни здравно-социални услуги до 2 часа на ден за период от 12 месеца и включва: - асистентска почасова подкрепа в дома за дейности, в които потребителят среща трудности (почистване на дома, пазаруване, готвене, придружаване, административни дейности, общуване, др.);- професионална подкрепа от здравен асистент/рехабилитатор и социален работник (информиране, консултиране, рехабилитационна подкрепа, в зависимост от потребностите и възможностите в домашна среда);- дистанционна подкрепа в ситуация на риск (чрез подаване на сигнал чрез телекеър оборудване, което води до връзка с мониторинг център и/ или посещение в дома на потребителя от посочено от потребителя лице).Предвидените услуги ще бъда предоставени на 50 потребители от всички населени места в Община Ивайловград: - Възрастни хора над 65 г. с ограничения или в невъзможност за самообслужване - 50 лица, включваща и Хора с увреждания и техните семейства- 8 лица;Община Ивайловград ще възложи услугата на общинско социално предприятие "Социален патронаж". За организиране и предоставяне на предвидените услуги ще бъдат ангажирани 10 лица, които ще преминат специализирани обучения за предлагане на мобилните здравно -социални услуги. (Bulgarian)

The aim of the project is to improve the quality of life and the opportunities for social inclusion of people with disabilities and elderly people from the Municipality of Ivaylovgrad by providing services in a home environment. The integrated service provides mobile health-social services up to 2 hours per day for a period of 12 months and includes: — hourly assistant support in the home for activities where the consumer encounters difficulties (cleaning the home, shopping, cooking, accompanying, administrative activities, communication, etc.);- professional support from a health assistant/rehabilitator and social worker (information, counselling, rehabilitation support, depending on the needs and opportunities in a home environment); — remote support in a situation of risk (by reporting via telecare equipment, which leads to a connection to a monitoring centre and/or a visit to the consumer’s home by a person designated by the user).The services provided will be provided to 50 users from all settlements in the Municipality of Ivaylovgrad: — Elderly people over 65 years old with restrictions or inability to self-service — 50 persons, including people with disabilities and their families — 8 persons; Municipality of Ivaylovgrad will entrust the service to the municipal social enterprise “Social patronage”. 10 persons will be involved in organising and providing the envisaged services, who will undergo specialised trainings for the provision of mobile health-social services. L’objectif du projet est d’améliorer la qualité de vie et les possibilités d’inclusion sociale des personnes handicapées et des personnes âgées de la municipalité d’Ivaylovgrad en fournissant des services dans un environnement familial. Der integrierte Service bietet mobile Gesundheits- und Sozialdienste bis zu 2 Stunden pro Tag für einen Zeitraum von 12 Monaten an und umfasst: — stündliche Assistenzunterstützung im Haus für Tätigkeiten, bei denen der Verbraucher Schwierigkeiten hat (Reinigung des Hauses, Einkaufen, Kochen, Begleitung, administrative Tätigkeiten, Kommunikation usw.);- professionelle Unterstützung durch einen Gesundheitsassistenten/Rehabilitator und Sozialarbeiter (Information, Beratung, Rehabilitationsunterstützung, je nach Bedarf und Möglichkeiten in einer häuslichen Umgebung); – Fernunterstützung in einer Risikosituation (durch Berichterstattung über Telecare-Geräte, die zu einer Verbindung zu einem Überwachungszentrum und/oder einem Besuch des Verbrauchers durch eine vom Nutzer benannte Person führt).Die angebotenen Dienstleistungen werden 50 Nutzern aus allen Siedlungen in der Gemeinde Ivaylovgrad angeboten: — Ältere Menschen über 65 Jahre mit Einschränkungen oder Unfähigkeit zur Selbstbedienung – 50 Personen, einschließlich Menschen mit Behinderungen und deren Familien – 8 Personen; Gemeinde Ivaylovgrad wird den Dienst dem kommunalen Sozialunternehmen „Soziale Schirmherrschaft“ anvertrauen. 10 Personen werden an der Organisation und Erbringung der geplanten Dienstleistungen beteiligt sein, die spezialisierte Schulungen für die Bereitstellung mobiler Gesundheits- und Sozialdienste absolvieren. El servicio integrado ofrece servicios móviles de salud y sociales de hasta 2 horas diarias durante un período de 12 meses e incluye: — ayudante por hora en el hogar para actividades en las que el consumidor encuentre dificultades (limpieza del hogar, compras, cocina, acompañamiento, actividades administrativas, comunicación, etc.);- apoyo profesional de un asistente sanitario/rehabilitador y trabajador social (información, asesoramiento, apoyo a la rehabilitación, en función de las necesidades y oportunidades en un entorno domiciliario); — apoyo a distancia en una situación de riesgo (por medio de informes a través de equipos de teleasistencia, lo que conduce a una conexión con un centro de seguimiento o una visita al hogar del consumidor por parte de una persona designada por el usuario). Los servicios prestados se prestarán a 50 usuarios de todos los asentamientos del municipio de Ivaylovgrad: — Personas mayores de 65 años con restricciones o incapacidad para el autoservicio — 50 personas, incluidas las personas con discapacidad y sus familias — 8 personas; el municipio de Ivaylovgrad confiará el servicio a la empresa social municipal «Mecenazgo social». 10 personas participarán en la organización y prestación de los servicios previstos, que recibirán formación especializada para la prestación de servicios sanitarios y sociales móviles.
