Our application for support relates to the development objectives/indicative allocation of resources allocated primarily to the district of Sarkadi, as set out in point 1.2 of the Specific Annex of the Area. In case of exhaustion of the allocated envelope, we intend to implement the development from the indicative allocation allocated to each county. The project proposal is located in Hungary, the South Great Plain Region, Békés county, the district of Sarkadi, in the village of Kötegyán. Annex 3 to Government Decree No 290/2014 (XI.26) and Annex 3 to Government Decree No 106/2015 (IV.23) is a settlement in Békés county classified as one of the beneficiary municipalities in districts to be developed with a complex programme on the basis of Annex 3 to Government Decree No 23/2001 (IV.23). The commune includes 105/2015 The municipalities benefiting from socio-economic and infrastructural aspects listed in Annex 3 to Government Decree (IV.23) (IV.23) are beneficiary municipalities. The municipality is included in 27/2013. Points 1 and 2 of Government Decree (II.12) (II.12) between the municipalities of the free enterprise zones. The settlement is characterised by unfavourable demographic trends, and due to emigration, ageing population and rising unemployment rates, the municipality has to devote an increasing amount to social expenditure. Every year, the number of people in need of regular social assistance and for elderly people and housing support is increasing. Its permanent population is the main population in 2015, which has been steadily decreasing since 2002. (source: (HCSO) The rate of reduction is a consequence of the mutually reinforcing effect of unfavourable demographic developments. The declining population of inhabitants is due to natural decline, ageing and migration losses that are characteristic of the whole decade and are clearly increasing towards the present. Exodus affects a higher proportion of those who are capable of reproduction. The unfavourable demographic indicators of Békés county emphasise the issue of the population retention power of settlements in the county, which the local government and local communities play a key role in preserving and strengthening. The health status of the population in Békés County is unfavourable in comparison to the country. This is supported by morbidity (patient) and mortality (morbidity) data. THAT’S 100. The number of deaths per 000 is close to 10 % above the national average. The most common causes of death are circulatory diseases, respiratory diseases, cancers, and nutritional and metabolic diseases (obesity, diabetes). The fact that the region’s mortality rates are much higher than the national average, while the health care access rates are low, indicates that most municipalities in the county, including Kötegyán, have access problems. The property is owned by the Municipality of Kötegyán Municipality of the Municipality of Kötegyán, which hosts the site of development and is completely free of litigation, burdens and demands. The building is located at the natural address at 5725 Kötegyán, Vasút u. 1. 1 GP, 1 dental and 1 protective female counsellor is at the disposal of the patients in the building where the primary health care to be developed is provided by a mixed general practitioner, a protective woman’s service and a dental practice. The nurse’s practice is currently replaced, so it’s unfilled. Taking into account point 3.2.1(d) of the call, the municipality undertakes in the Grant Agreement, on pain of repayment of the aid, that at the time of submission of the project, the protective service concerned by the development will be filled by the project closure. In 2012, in the framework of the DAOP-4.1.1/C-09 tender, the external and internal refurbishment, the complete accessibility of the building, the refurbishment of the interior rooms and the replacement and extension of the equipment, furniture and computers were carried out in the framework of tender DAOP-4.1.1/C-09. On the tasks, ideas and development needs related to the development, the local government agreed with the general practitioner, dentist and protective woman. The supporting statement was attached to the grant application. The useful floor area of the building is 293.73 m², of which the waiting rooms are 69.39 m², clinics and auxiliary rooms have an area of 155.77 m². The renovation and modernisation of the roofing structure of the property is justified, as this could not be achieved from the previous tender. The project involves the modernisation of the roofing structure of the building and the replacement of tiles. Támogatási kérelmünk a elsődlegesen a Terület Specifikus Melléklet 1.2 pontjában szereplő Sarkadi járás számára elkülönített fejlesztési célok/forrásfelhasználási indikatív keretösszegre irányul, Az elkülönített keretösszeg kimerülése esetén a fejlesztést, a minden megyén belül jogosult számára elkülönített indikítív keretösszeg terhére kívánjuk megvalósítani. A projektjavaslat megvalósulásának helyszíne Magyarországon, a Dél-Alföldi Régióban, Békés megyében, a Sarkadi járásban, Kötegyán Községben található. Kötegyán a 290/2014 (XI.26) Korm. rendelet 3. sz. melléklete, valamint a 106/2015. (IV.23) Korm. rendelet 3. sz. melléklete alapján komplex programmal fejlesztendő járásokban található kedvezményezett települések közé sorolt település Békés megyében. A község beletartozik a 105/2015. (IV.23) Korm. rendelet 3. sz. mellékletében felsorolt társadalmi-gazdasági és infrastrukturális szempontból kedvezményezett települések kedvezményezett települések közé. A település beletartozik a 27/2013. (II.12) Korm. rendelet 1. és 2. pontjában megtalálható szabad vállalkozási zónák települései közé. A települést kedvezőtlen demográfiai folyamatok jellemzik, az elvándorlás, a lakosság elöregedése, a munkanélküliségi ráta emelkedése miatt az önkormányzatnak egyre nagyobb összeget kell szánnia a szociális kiadásokra. Évről évre növekszik a rendszeres szociális segélyre szorulók, illetve az időskorúak ellátásában, lakásfenntartási támogatásban részesülők száma. Állandó lakosságának száma 2015-ben fő, mely 2002 óta folyamatosan csökken. (forrás: KSH) A csökkenés üteme a kedvezőtlen demográfiai folyamatok egymást erősítő hatásának következménye. A csökkenő lakónépesség az évtized egészére jellemző és a jelen felé egyértelműen erősödő természetes fogyásra, elöregedésre és vándorlási veszteségre vezethető vissza. Az elvándorlás a magasabb arányban érinti a reprodukcióra képes korosztályokat. Békés megye kedvezőtlen demográfiai mutatói különösen hangsúlyossá teszik a megye településeinek népességmegtartó erejének kérdését, melynek megőrzésében, erősítésében a helyi önkormányzatnak, helyi közösségeknek kiemelt szerepe van. Békés megyében a népesség egészségi állapota országos összehasonlításban kedvezőtlen. Ezt támasztják alá a morbiditási (megbetegedési) és mortalitási (halálozási) adatok is. A 100. 000 főre eső halálozások száma az országos átlagot közel 10%-kal haladja meg. A leggyakoribb halálokként a keringési megbetegedéseket, a légző-rendszer betegségeit, a daganatos megbetegedéseket, valamint a táplálkozási és anyagcsere (elhízás, diabétesz) betegségeket tartják számon. Az a tény, hogy a térség halálozási mutatói az országos átlagnál jóval magasabbak, ugyanakkor az egészségügyi ellátás igénybevételi mutatói alacsonyak, azt jelzi, hogy a megye legtöbb települése, így Kötegyán is hozzáférési problémával küzd. A fejlesztés helyszínéül szolgáló 639/1 helyrajzi számú Egészségháznak helyt adó ingatlan Kötegyán Község Önkormányzatának tulajdona, mely teljes mértékben per-, teher- és igénymentes. Az épület a 5725 Kötegyán, Vasút u. 1. szám alatti természetes címen található. A fejlesztendő egészségügyi alapellátást biztosító vegyes háziorvosi, védőnői szolgálatnak és a fogorvosi rendelőnek helyt adó épületben 1 háziorvos, 1 fogorvosi és 1 védőnői tanácsadó áll a betegek rendelkezésére. A védőnői praxis jelenleg helyettesítéssel van ellátva, tehát betöltetlen. Az önkormányzat a felhívás 3.2.1.d) pontjában leírtakat figyelembe véve a Támogatási Szerződésben a támogatás visszafizetésének terhe mellett vállalja, hogy a projekt benyújtásának időpontjában a fejlesztéssel érintett védőnői szolgálat a projektzárásig betöltésre kerül. 2012-ben DAOP-4.1.1/C-09 pályázat keretében megvalósult a háziorvosi, fogorvosi és védőnői rendelőt magába foglaló épület külső és belső felújítása, teljes körű akadálymentesítése, a belső helyiségek átalakítása, illetve az eszközállomány, a bútorzat és a számítógépek cseréje, bővítése. A fejlesztéssel kapcsolatos feladatokról, elképzelésekről, a fejlesztési igényekről az önkormányzat egyeztetett a háziorvossal, fogorvossal, védőnővel, melyet alátámasztó nyilatkozat a támogatási kérelemhez csatolásra került. Az épület hasznos alapterülete 293,73 m2, melyből a várótermek 69,39 m2, rendelők és kiegészítő helyiségek 155,77 m2 alapterületűek. Az ingatlan tetőszerkezetének felújítása, korszerűsítése indokolt, mivel ez az előző pályázatból nem tudott megvalósulni. A projekt keretében megtörténik az épület tetőszerkezetének korszerűsítése, cserepek cseréje. Felújításra kerül az egész épület tetőszerkezete, mely 1 fogorvosi rendelő, 1 védőnői tanácsadó, 1 háziorvosi rendelő és a hozzájuk kapcsolódó 24 kiszolgáló helyiséghez kapcsolódik. A fa tetőszerkezet csabai hornyolt cserépfedéssel készült az 1970-es években, a korra jellemző technológiai megoldásokkal.
