Nowadays, ultrasound diagnostics provide essential information for medicine. It is essential to make a diagnosis, influences therapeutic decisions, is a basic, often first choice method in the follow-up of diseases. The B.A.Z. County Central Hospital’s ultrasound apparatus, according to its development, is lagging behind compared to today’s requirements and priority position. Only two devices from the ultrasound machine park are among the highly knowledgeable, modern software equipment. Due to the numerous outdated, sometimes technically and professionally invaluable quality equipment, the existing limited number of devices with good parameters are subject to increased stress, which sometimes also results in “hipping” of technology. Therefore, these modern devices fail more than expected. In our hospital we need to develop our ultrasound equipment to ensure the highest level of patient care (III.progression) in many professions corresponding to our central position and to ensure the development of professionals. One of the two ultrasound equipment to be procured is for cardiology lying care, where proper diagnostics are essential during emergency and invasive care. The other is a technological leap in the field of treatment with gynecological inpatient care, where we would use these equipment to examine patients in the perioperative period, which is essential for the proper performance of surgery. Proper diagnostics are essential both in the pre- and postoperative phases of surgeries, and proper diagnostics can shorten the duration of hospital care. Purchase of 1 Pcs Cardiological Ultrasound Equipment: One of the diagnostic pillars of acute cardiological care and intensive therapy is emergency echocardiography, which is not possible without an ultrasound (UH) device of sufficient quality. It is not possible to transport patients in haemodynamically unstable condition, ventilated patients in the perioperative period to conventional ultrasound laboratories; large ultrasound workstations continuously serving outpatient care in specialised clinics can also not be moved next to the patient bed or operating table. UH diagnostics are also essential for the facilitation of certain interventions (e.g. UH-controlled large vein cannulation, pericardiocentesis). Due to the high intensive care unit traffic, regional patient transfers, definitive decisions affecting the patient’s fate should be based on bedside studies (e.g. aortic dissection, urgent heart surgery in case of mechanical infarction complication). In patients with critical conditions, imaging is often difficult (bed back situation, expanded lungs due to ventilating, patient lack of cooperation), which is why it is important to use the best quality transducers and data analysis software. 1pc Acquisition of high-level obstetric and gynaecological ultrasound: The equipment is placed in the Department of Obstetrics and Gynaecology of the Member Hospital of Semmelweis requesting support. The obstetric, gynecological and oncological care area of the department concerned is Miskolc city, Miskolc surrounding settlements, Tiszaújvárosi, Mezőcsáti and Mezőkövesdi districts. The annual number of births in the department has been 2100-2200 for years, 5,623 in-patient patients were cared for in 2016 and 26110 ultrasound examinations were carried out in the department in 2016. About one third of these studies are complex UH examinations: — ultrasound examination of inpatient patients awaiting or undergoing oncological and gynaecological surgery, — completion of parent room monitoring with ultrasound examination, — observation of endangered pregnant mothers lying on pregnancy pathology by ultrasound examination to reduce fetal morbidity and mortality, — newborn ultrasound examination. Given that we are the Center for Oncology, ultrasound diagnostics have a significant role to play in the investigation and follow-up of oncology patients. The equipment to be procured under the project is exclusively related to the provision of public inpatient healthcare. No additional funding is required for improvements. For the professions developed, the applicant has a funding contract. The applicant shall undertake, at its own cost, to carry out all mandatory non-eligible activities referred to in the call for proposals. Project management tasks are carried out by a staff member of the hospital’s Tender and Project Group. Procurement tasks are carried out by staff of the procurement team of the hospital. The mandatory elements of the communication package according to the Beneficiary’s Information Obligations Guide (Széchenyi 2020 Arculati Manual) will be implemented at our own expense. Napjainkban az ultrahang diagnosztika az orvostudomány számára esszenciális információkat nyújt. Nélkülözhetetlen a diagnózis felállításához, befolyásolja a terápiás döntéseket, alapvető, sokszor először választandó módszer a betegségek követésében. A B.A.Z. Megyei Központi Kórház ultrahang készülék - fejlettség szerinti - ellátottsága a mai kor követelményeihez és kiemelt pozíciójához képest elmarad. Az ultrahang gépparkból mindössze két készülék tartozik a nagy tudású, korszerű szoftvereket tartalmazó berendezések közé. A számos elavult, esetenként technikailag és szakmailag értékelhetetlen minőségű eredményt szolgáltató berendezés miatt a meglévő kevés számú, jó paraméterekkel rendelkező készülékek fokozott igénybevételnek vannak kitéve, amely igénybevétel esetenként a technológia „lerablását” is eléri. Ezért ezek a modern készülékek az elvártnál többször meghibásodnak. Kórházunkban a központi pozíciónknak megfelelő számos szakmában a legmagasabb (III.progresszvitási) szintű betegellátás, a szakemberek fejlődésének biztosítása érdekében ultrahang eszközparkunkat fejlesztenünk kell. A két beszerzésre kerülő ultrahang berendezés közül az egyik a kardiológia fekvő ellátáshoz kerül beszerzésre, ahol a sürgősségi és invazív ellátás során elengedhetetlen a megfelelő diagnosztika. A másik a nőgyógyászati fekvőbeteg ellátás tekintetében jelent technológiai ugrást a gyógyítás területén, ahol a perioperatív időszakban lévő betegek vizsgálatára használnánk ezen berendezéseket, mely a műtétek megfelelő elvégzéséhez elengedhetetlen. A megfelelő diagnosztika a műtétek pre és postoperatív szakaszában is elengedhetetlen, a megfelelő színvonalú diagnosztika lerövidítheti a kórházi ellátás időtartamát. 1 db Emelt szintű kardiológiai ultrahang berendezés beszerzése: Az akut kardiológiai ellátás és intenzív terápia egyik diagnosztikus alappillérje a sürgősségi echocardiographia, mely nem lehetséges megfelelő minőségű ultrahang (UH) készülék nélkül. A haemodinamikailag instabil állapotú, a lélegeztetett, a perioperatív időszakban lévő betegek szállítása a konvencionális ultrahang laboratóriumokba nem lehetséges; a nagyméretű, szakrendeléseken folyamatosan a járóbetegellátás szolgálatában álló ultrahang munkaállomások pedig szintén nem mozgathatók a betegágy vagy műtőasztal mellé. Nélkülözhetetlen továbbá az UH diagnosztika egyes intervenciók facilitálására (pl. UH vezérelt nagyvéna kanülálás, pericardiocentesis). A nagy intenzív osztályos betegforgalom, regionális betegáthelyezések miatt definitív, a beteg sorsát érintő döntéseket kell alapozni ágy mellett végzett vizsgálatokra (pl. aorta disszekció, mechanikus infarctusos szövődmény esetén sürgős szívműtét). A kritikus állapotú betegekben gyakran nehéz a képalkotás (ágyban háton fekvő helyzet, lélegeztetés miatt expandált tüdők, beteg együttműködésének hiánya), ezért is fontos a legjobb minőségű transzducerek és adatelemző szoftverek használata. 1db Kiemelt szintű szülészeti-nőgyógyászati ultrahang beszerzése: A berendezés a támogatást igénylő Semmelweis Tagkórházának Szülészeti és Nőgyógyászati Osztályán kerül elhelyezésre. Az érintett osztály szülészeti, nőgyógyászati és onkológiai ellátási területe Miskolc város, Miskolc környéki települések, Tiszaújvárosi, Mezőcsáti és Mezőkövesdi járás. Az osztályon évek óta 2100-2200 az éves szülésszám, 2016 évben 5623 fekvőbeteget ápoltak és 2016-ban az osztályon 26110 ultrahang vizsgálat történt. Ezen vizsgálatok kb. egyharmada összetett, komplex UH vizsgálat:- az onkológiai és nőgyógyászati műtétre váró, illetve műtéten átesett fekvőbetegeink ultrahang vizsgálata, - szülőszobai monitorizálás kiegészítése ultrahang vizsgálattal, - a terhespathologián fekvő, veszélyeztetett terhes kismamák observatioja ultrahang vizsgálattal a magzati morbiditás, mortalitás csökkentése érdekében, - újszülött ultrahang vizsgálat. Tekintettel arra, hogy Onkológiai Központ vagyunk, az onkológiai betegek kivizsgálása és nyomon követése tekintetében az ultrahang diagnosztikára jelentős szerep hárul. A projekt keretében beszerzésre kerülő eszközök kizárólag közfinanszírozott fekvőbeteg egészségügyi ellátás nyújtásához kapcsolódnak. A fejlesztések többletfinanszírozást nem igényelnek. A fejlesztett szakmák vonatkozásában a pályázó finanszírozási szerződéssel rendelkezik. A támogatást igénylő a pályázati felhívásban szereplő valamennyi kötelezően megvalósítandó, önállóan nem támogatható tevékenység megvalósítását saját költségén vállalja. A projektmenedzsment feladatokat a kórház Pályázati és Projekt csoportjának munkatársa látja el. A közbeszerzési feladatokat a kórház beszerzési csoportjának munkatársai végzik. A kedvezményezett tájékoztatási kötelezettségei útmutató szerinti (Széchenyi 2020 Arculati Kézikönyv) kommunikációs csomag kötelező elemeit saját költségen megvalósítjuk. (Hungarian)
