Основната цел на проектното предложение и да се изгради модел за почасови мобилни интегрирани здравно-социални услуги – патронажна грижа за нуждаещи се лица с увреждания и възрастни хора в община Асеновград за подобряване качеството на живот и възможностите им за социално включване.Община Асеновград ще осигури подходящи помещения, на които ще бъде извършен ремонт и ще се закупи оборудване и обзавеждане. Ще се закупи автомобил за осигуряване на мобилността на услугата. Ще бъде нает екип от специалисти (медицински сестри, домашни санитари, социални работници и други), които ще предоставят различни здравни и социални дейности за подкрепа. Предвидена е подкрепа на персонала, ангажиран пряко с предоставянето на здравно-социалните услуги – обучения и супервизия.При реализирането на дейностите ще бъдат обхванати и ще се осигури достъп до дългосрочна мобилна и интегрирана здравно-социална грижа, според специфичните потребности на 168 лица от целевите групи, а именно: Възрастни хора над 65 г. с ограничения или в невъзможност за самообслужване; Хора с увреждания и техните семейства. Те ще получат и психологическа подкрепа и консултиране. Патронажната грижа ще се предоставя 12 месеца в рамките на проекта, а след неговото приключване общината ще търси други възможности за финансиране.Проектното предложение е разработено в съответствие с Мярка 2 (операция 6): Осигуряване на патронажна грижа чрез предоставяне на почасови мобилни интегрирани здравно-социални услуги за нуждаещите се лица с увреждания и възрастни хора на Плана за действие за периода 2018 – 2021 г. за изпълнение на Националната стратегия за дългосрочна грижа, приет с РМС № 28/19.01.2018 г. и е част от цялостния процес на деинституционализация на грижата за хората с уреждания и възрастните хора и изграждането на алтернативна подкрепа в общността чрез предоставянето на качествени и достъпни услуги за дългосрочна грижа. The main objective of the project proposal and to build a model for hourly mobile integrated health and social services — patronage care for deprived persons with disabilities and elderly people in the municipality of Asenovgrad to improve the quality of life and their opportunities for social inclusion. Municipality of Asenovgrad will provide suitable premises for which repairs will be carried out and equipment and furniture will be purchased. A car will be purchased to ensure the mobility of the service. A team of specialists (nurses, domestic orderlies, social workers, etc.) will be recruited to provide various health and social support activities. Support for the staff directly involved in the provision of health and social services — trainings and supervision is envisaged. In the implementation of the activities will be covered and will provide access to long-term mobile and integrated health-social care, according to the specific needs of 168 persons from the target groups, namely: Elderly people over 65 years of age with restrictions or inability to self-service; People with disabilities and their families. They will also receive psychological support and counselling. Patronage care will be provided 12 months within the project, and after its completion the municipality will look for other funding opportunities.The project proposal is developed in accordance with Measure 2 (Operation 6): Provision of patronage care through the provision of hourly mobile integrated health and social services for deprived persons with disabilities and the elderly of the Action Plan 2018-2021 for the implementation of the National Strategy for Long-term Care, adopted by the Council of Ministers No 28/19.01.2018 and is part of the overall process of deinstitutionalisation of care for people with regulated and elderly people and building alternative support in the community through the provision of quality and affordable long-term care services. L’objectif principal de la proposition de projet et de construire un modèle de services sociaux et de santé intégrés mobiles horaires — soins de mécénat pour les personnes handicapées et les personnes âgées dans la municipalité d’Asenovgrad afin d’améliorer la qualité de vie et leurs possibilités d’inclusion sociale. La municipalité d’Asenovgrad fournira des locaux appropriés pour lesquels des réparations seront effectuées et des équipements et du mobilier seront achetés. Une voiture sera achetée pour assurer la mobilité du service. Une équipe de spécialistes (infirmières, domestiques, travailleurs sociaux, etc.) sera recrutée pour fournir diverses activités de santé et de soutien social. Das Hauptziel des Projektvorschlags und die Entwicklung eines Modells für die integrierte Gesundheits- und Sozialdienste pro Stunde – Betreuung von benachteiligten Menschen mit Behinderungen und älteren Menschen in der Gemeinde Asenovgrad zur Verbesserung der Lebensqualität und der Möglichkeiten der sozialen Eingliederung. Die Gemeinde Asenovgrad wird geeignete Räumlichkeiten zur Verfügung stellen, für die Reparaturen durchgeführt und Ausrüstung und Möbel gekauft werden. Ein Auto wird gekauft, um die Mobilität des Dienstes zu gewährleisten. Ein Team von Spezialisten (Krankenpfleger, Hausangestellte, Sozialarbeiter usw.) wird eingestellt, um verschiedene Gesundheits- und Sozialhilfeaktivitäten anzubieten. Unterstützung des Personals, das direkt an der Erbringung von Gesundheits- und Sozialdienstleistungen beteiligt ist – Schulungen und Aufsichtsmaßnahmen sind vorgesehen. De hoofddoelstelling van het projectvoorstel en het ontwikkelen van een model voor mobiele, geïntegreerde gezondheids- en sociale diensten per uur — beschermheerschapszorg voor behoeftigen met een handicap en ouderen in de gemeente Asenovgrad om de kwaliteit van leven en hun kansen op sociale inclusie te verbeteren. De gemeente Asenovgrad zal geschikte gebouwen bieden waarvoor reparaties zullen worden uitgevoerd en apparatuur en meubilair zullen worden gekocht. Er wordt een auto gekocht om de mobiliteit van de dienst te waarborgen. Een team van specialisten (verpleegsters, huisartsen, maatschappelijk werkers, enz.) zal worden aangeworven om diverse gezondheids- en sociale ondersteunende activiteiten te verrichten. Ondersteuning van het personeel dat rechtstreeks betrokken is bij de verstrekking van gezondheids- en sociale diensten — opleidingen en toezicht zijn gepland. Bij de uitvoering van de activiteiten zal worden voorzien in toegang tot mobiele en geïntegreerde gezondheidszorg op lange termijn, afhankelijk van de specifieke behoeften van 168 personen uit de doelgroepen, namelijk: Ouderen ouder dan 65 jaar met beperkingen of onvermogen tot zelfbediening; Mensen met een handicap en hun gezinnen. Zij zullen ook psychologische ondersteuning en begeleiding krijgen. Beschermheerschapszorg wordt 12 maanden binnen het project verstrekt, en na de voltooiing ervan zal de gemeente op zoek gaan naar andere financieringsmogelijkheden.Het projectvoorstel wordt ontwikkeld in overeenstemming met maatregel 2 (operatie 6): Het verlenen van beschermheerschapszorg door het verlenen van mobiele geïntegreerde gezondheids- en sociale diensten per uur voor behoeftigen met een handicap en ouderen van het actieplan 2018-2021 voor de uitvoering van de nationale strategie voor langdurige zorg, aangenomen door de ministerraad nr. 28/19.01.2018, en maakt deel uit van het algemene proces van deïnstitutionalisering van de zorg voor mensen met gereguleerde en ouderen en het opbouwen van alternatieve ondersteuning in de gemeenschap door middel van hoogwaardige en betaalbare langdurige zorg. L'obiettivo principale della proposta di progetto e di costruire un modello per i servizi sanitari e sociali integrati mobili orari — assistenza al patrocinio per le persone con disabilità e gli anziani nel comune di Asenovgrad per migliorare la qualità della vita e le loro opportunità di inclusione sociale. Verrà acquistata un'auto per garantire la mobilità del servizio. Un team di specialisti (infermieri, inservienti domestici, assistenti sociali, ecc.) sarà assunto per fornire varie attività di assistenza sanitaria e sociale. Sostegno al personale direttamente coinvolto nella prestazione di servizi sanitari e sociali — è prevista la formazione e la supervisione. Nell'attuazione delle attività sarà coperto e consentirà l'accesso a cure sanitarie-sociali mobili e integrate a lungo termine, in funzione delle esigenze specifiche di 168 persone provenienti dai gruppi destinatari, vale a dire: Persone anziane di età superiore ai 65 anni con restrizioni o incapacità al self-service; Le persone con disabilità e le loro famiglie. Riceveranno anche sostegno psicologico e consulenza. Il patrocinio sarà fornito 12 mesi all'interno del progetto, e dopo il suo completamento il comune cercherà altre opportunità di finanziamento. La proposta di progetto è sviluppata in conformità con la Misura 2 (Operazione 6): Prestazione di assistenza al patrocinio attraverso la fornitura di servizi sanitari e sociali integrati mobili orari per le persone indigenti con disabilità e gli anziani del piano d'azione 2018-2021 per l'attuazione della strategia nazionale per l'assistenza a lungo termine, adottato dal Consiglio dei ministri n. 28/19.1.2018 e rientra nel processo globale di deistituzionalizzazione dell'assistenza alle persone con persone regolamentate e anziane e la costruzione di un sostegno alternativo nella comunità attraverso la fornitura di servizi di assistenza a lungo termine di qualità e a prezzi accessibili. El objetivo principal de la propuesta de proyecto y la construcción de un modelo de servicios sanitarios y sociales integrados móviles por hora — la atención de patrocinio a las personas desfavorecidas con discapacidad y las personas mayores en el municipio de Asenovgrad para mejorar la calidad de vida y sus oportunidades de inclusión social; el municipio de Asenovgrad proporcionará locales adecuados para las reparaciones y se adquirirán equipos y mobiliario. Se adquirirá un coche para garantizar la movilidad del servicio. Se contratará un equipo de especialistas (enfermeras, ordenanzas domésticas, trabajadores sociales, etc.) para que realicen diversas actividades de apoyo sanitario y social. Apoyo al personal directamente implicado en la prestación de servicios sanitarios y sociales — se prevé la formación y supervisión.
