Grupę docelową stanowi 60 os.(36K,24M)osoby bezrobotne to jest 42 osoby i osoby bierne zawodowe to jest 18 osób zagrożonych ubóstwem lub wykluczeniem społecznym, doświadczających wielokrotnego wykluczenia społecznego rozumianego jako wykluczenie z powodu więcej niż 1 z przesłanek tj.: •osoby korzystające ze świadczeń z pomocy społecznej zgodnie z ustawą z dnia 12.03.2004r.o pomocy społecznej lub kwalifikujące się do korzystania ze świadczeń z pomocy społecznej to jest spełniające co najmniej jedną z przesłanek określonych w artykule 7 ustawy z dnia 12.03.2004r., •osoby korzystające z Programu Operacyjnego Pomoc Żywnościowa, •osoby zakwalifikowane do III profilu pomocy, zgodnie z ustawą z dnia 20.04.2004r.o promocji zatrudnienia i instytucjach rynku pracy, nie więcej niż 20% Uczestników Projektu to jest 12 osób, •osoby z niepełnosprawnością- minimum 30%Uczestników Projektu to jest 18 osób, zamieszkujących w rozumieniu Kodeksu Cywilnego na terenie woj.mazowieckiego w gminach poniżej progu defaworyzacji określonego w Mazowieckim barometrze ubóstwa i wykluczenia społecznego opisanych w pkt C.1.7 wniosku. Celem głównym projektu jest podwyższenie aktywności społeczno-zawodowej wyżej wymienionej grupy docelowej poprzez realizację zadań:(diagnozy indywidualnych potrzeb i potencjałów UP,poradnictwa psychologicznego, pośrednictwo pracy, treningu skutecznej komunikacji, warsztatów rozwoju osobistego,szkoleń i staży) od 01.08.2018r. do 31.07.2019r. prowadzącą do zatrudnienia minimum 18 osób oraz postępu w aktywizacji społecznej minimum 21 osób. W projekcie zostały opracowane następujące rezultaty: Liczba osób zagrożonych ubóstwem lub wykluczeniem społecznym, które uzyskały kwalifikacje po opuszczeniu programu-14 osób. -Liczba osób zagrożonych ubóstwem lub wykluczeniem społecznym poszukujących pracy po opuszczeniu programu-34 osoby. The target group consists of 60 people (36K,24M) unemployed persons, i.e. 42 persons and inactive persons, i.e. 18 people at risk of poverty or social exclusion, experiencing multiple social exclusion understood as exclusion due to more than 1 of the indications: • Persons benefiting from social assistance benefits in accordance with the Act of 12.03.2004r.o. on social assistance or eligible for social assistance benefits is meeting at least one of the conditions specified in Article 7 of the Act of 12.03.2004, • persons benefiting from the Operational Programme Food Aid, • persons qualified to the third profile of the aid, according to the Law of 20.04.2004r on promotion of employment and labour market institutions, is not more than 20 % of the participants in the project. The main objective of the project is to increase the socio-occupational activity of the above-mentioned target group through the implementation of tasks: (diagnosis of individual needs and potentials of the University, psychological advice, job mediation, effective communication training, personal development workshops, trainings and traineeships) from 01.08.2018 to 31.07.2019, leading to employment of at least 18 people and progress in social activation of at least 21 people. The project produced the following results: Number of people at risk of poverty or social exclusion who have been qualified after leaving programme-14. —Number of people at risk of poverty or social exclusion seeking work after leaving programme-34. Le groupe cible est de 60 personnes (36K,24 millions) au chômage, c’est-à-dire 42 personnes et inactives, soit 18 personnes exposées au risque de pauvreté ou d’exclusion sociale, confrontées à une exclusion sociale multiple, comprise comme une exclusion due à plus d’une des conditions, à savoir: •les bénéficiaires de prestations d’assistance sociale conformément à la loi du 12.03.2004 sur l’assistance sociale ou ayant droit aux prestations d’assistance sociale sont qu’ils remplissent au moins l’une des conditions prévues à l’article 7 de la loi du 12.03.2004, •les bénéficiaires du programme opérationnel d’aide alimentaire, les personnes qualifiées du troisième profil d’aide, conformément à la loi du 20.04.2004 sur la promotion de l’emploi et des institutions du marché du travail, 20 % au maximum des participants au projet sont 12 personnes, •les personnes handicapées — au moins 30 % des participants au projet sont 18 personnes au sens du Code civil au sens du Code civil. L’objectif principal du projet est d’accroître l’activité socioprofessionnelle du groupe cible susmentionné grâce à la mise en œuvre de tâches (diagnostic des besoins et des potentiels individuels de l’UP, conseil psychologique, médiation professionnelle, formation à la communication efficace, ateliers de développement personnel, formations et stages) du 01.08.2018 au 31.7.2019. conduisant à l’emploi d’un minimum de 18 personnes et à progresser dans l’activation sociale d’un minimum de 21 personnes. Les résultats suivants ont été élaborés dans le cadre du projet: Nombre de personnes exposées au risque de pauvreté ou d’exclusion sociale ayant obtenu des qualifications après avoir quitté le programme-14. —Nombre de personnes menacées de pauvreté ou d’exclusion sociale à la recherche d’un emploi après avoir quitté le programme-34.
