През последните години значително се увеличи броя на хората, които поради една или друга причина - здравословен проблем, бедност, недостиг на средства се нуждаят от определени социални и здравни грижи. Проектното предложение предвижда предоставяне на дългосрочни грижи за лица с хронични заболявания, с трайни увреждания или самотно живеещи възрастни хора. Моделът за интегрирани здравно-социални услуги в домашна среда ще осигури медико-социална подкрепа от медицински специалисти и домашни помощници, а услугите ще се предоставят на база индивидуална оценка на всеки човек. Очакванията при реализиране на проекта са възрастните хора и тези с увреждания да живеят в собствената си среда и в същото време да получават грижата, от която се нуждаят. В повечето случаи това са хора с хронични заболявания, останали сами в своите домове. Домашните грижи, предоставяни в рамките на проекта, не са просто механичен сбор от медицински грижи и социални услуги, а представляват един нов подход, позволяващ въпреки болестите и другите несгоди, човек да остарее достойно в собствения си дом – там където са концентрирани спомените от целия му живот! Дейностите са насочени изцяло към възрастните хора над 65 години, с ограничения или невъзможност за самообслужване, както и към хора с трайни увреждания и техните семейства, към хора , които нямат близки и имат социална необходимост от почасови услуги. Ще се осигури комплекс от здравно-социални услуги и подкрепа в домашна среда за 50 потребителя. Мобилната услуга включва и дейности, насочени към подобряване качеството на живот на възрастните хора и тези с увреждания, в зависимост от конкретното състояние и ограничения. Чрез изпълнението на дейностите ще се подобри качеството на живот и възможностите за социално включване на хората с увреждания и възрастните хора, чрез осигуряване на мрежа от услуги в домашна среда и изграждане на подходящ(материален и кадрови) капацитет за предоставянето им. In recent years, there has been a significant increase in the number of people who, for one reason or another — a health problem, poverty, a shortage of resources — need certain social and health care. The project proposal provides for the provision of long-term care for people with chronic diseases, permanent disabilities or lonely elderly people. The model for integrated health-social services in the home environment will provide medical and social support from healthcare professionals and domestic workers, and the services will be provided on the basis of an individual assessment of each person. The expectations when implementing the project are that older people and those with disabilities will live in their own environment and at the same time receive the care they need. In most cases, these are people with chronic diseases left alone in their homes. Home care provided under the project is not just a mechanical sum of medical care and social services, but is a new approach, allowing a person to grow old dignifiedly in his own home — where the memories of his whole life are concentrated! The activities are aimed entirely at older people over 65, with limitations or inability to self-service, as well as people with permanent disabilities and their families, people who are not close and have a social need for hourly services. A complex of health-social services and support in a home environment will be provided for 50 users. The mobile service also includes activities aimed at improving the quality of life of older people and those with disabilities, depending on the specific situation and constraints. The implementation of the activities will improve the quality of life and opportunities for social inclusion of people with disabilities and the elderly, by providing a network of services in the home environment and by building appropriate (material and personnel) capacity to provide them.
